Job Summary
The RegisteredNursing (RN) is responsible to perform a competent level of nursing care as demonstrated by the critical thinking model known as the nursing process. The standards of practice (assessment, diagnosis, outcomes identification, planning, implementation, coordination of care- teaching and health promotion and evaluation) along with the standards of professional performance (ethics, culturally congruent practice, communication, collaboration, leadership, education, evidence- based practice /nursing research, quality, evaluation, resource utilization, and environmental health) encompass the actions and foundation of professional nursing. The RN possesses clinical knowledge and skills to meet standards as required by specific clinical areas.
BSN or MSN preferred. New Hire Education Requirement: All new RNs who do not have their BSN will be required to sign a BSN Agreement committing to enrollment in an accredited RN to BSN program within 24 months of hire and successfully obtaining their BSN within 5 years of hire. Meets and maintains any requirements defined by specific specialty; Critical Care/IMCU ACLS within 1 year of hire; Emergency Care - ACLS & PALS or ENPC within 1 year of hire; Oncology Care ONS/ONCC or Sentara approved course Chemotherapy & Biotherapy Provider Card within 6 months of hire. We are looking for an experienced nurse willing to work PRN to lead a team of caregivers, conduct in home assessments, and develop care plans for clients.
DUTIES OF POSITION
Maintains the quality of client care/services in accordance with the Agencys objectives and policies, through planning, coordination, and implementation and evaluation of the home services provided.
POSITION RESPONSIBILITIES
Reviews the clinical content, medical history, medications, and other procedures from the records of clients
Identifies and evaluates client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff
Develop the Plan of Care/Service after assessing the client and consulting the client and/or family member/responsible person
Review and update Care/Service Plans per policy
Sees clients in the home setting as needed
Participates in client record review
Performs supervisory visits per policy
JOB CONDITIONS
Travel is required to and from client's home when supervision of care/services is scheduled
The ability to communicate well, both verbally and in writing is required
The ability to access clients homes which may not be routinely wheelchair accessible is required
Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care/services
QUALIFICATIONS
Must be a RegisteredNurse licensed to practice in the state of Virginia
Experience in the provision of Personal Care Services or Facility Care / Assisted Living Care is preferably but not required
