Effectively uses the Nursing Process in the delivery of patient care. Assesses learning needs and implements teaching strategies appropriate for the diverse needs of the patient, family, and other groups or disciplines. Communicates, delegates and manages nursing team resources (human and fiscal) properly and serves as a leader and partner on the Interdisciplinary Team. Utilizes data, information, and knowledge to evaluate and promote change to achieve optimal outcomes. Embraces concepts and behaviors that enhance customer satisfaction and employee morale and improvement in the profession of nursing.

In this position every day brings new and energizing challenges. You will average 25-30 visits per week and with those visits your responsibilities range from assessing your patients and establishing a plan of care engaging the patient and/or family in the process, communicating expectations to other members of the nursing care team and initiating discharge planning upon admission utilizing appropriate resources.

To ensure you have the resources you need, all RN's are provided with a company car for visits, a laptop for documenting while on the go, a car wash pass, and a company phone.

Qualifications:
Experience
and education minimum requirements:


Graduation from a School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure. One (1) year acute care experience. Current California Registered Nurse License. Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA). Valid California driver's license and driving record acceptable to Community Medical Centers.

• 5 years of RN experience in an acute clinical setting, with at least one (1) year of wound care experience.
• Home Health Experience is recommended but not requirement.

SKILLS AND KNOWLEDGE

• Demonstrated clinical expertise in the area of wound care, including a working knowledge of pharmacology, medical terminology, and aseptic technique.
• Demonstrated general knowledge and competence with regard to nursing theories, concepts and practices; medical terminology; anatomy; and physiology.
• Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
• Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
• Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record.
• Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.
• Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
• Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.
• Ensure the privacy of each patient's Protected Health Information (PHI).
• Build collaborate relationships with peers and other healthcare providers to achieve departmental and corporate objectives.
