Responsibilities:
The Care Management Nurse, MDS Nurse works the RAI process and conducts assessments and care plan coordination for those residents assigned. Some responsibilities of the MDS nurse include:
Completes the MDS, CAA's and care plans within regulated time frames.
Assesses resident through physical assessment, interview and chart review.
Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
Coordinates, identifies, and/or initiates significant change MDS'
Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications:
RegisteredNurse, RN or Licensed Practical Nurse
AANC certification a plus. RAC-CT
Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.

2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting
1 year of leadership experience in a healthcare setting

JOB DESCRIPTION
The Detroit Medical Center (DMC) RegisteredNurse (RN) is a member of the Patient Care Services Team, and is responsible for the satisfactory completion of nursing care by the nursing team. The RN manages and provides patient care activities for a group of patients and their families through application of independent judgment, communication and collaboration with all team members including ancillary and support services. The role of the RegisteredNurse at the DMC encompasses leadership, partnership, collaboration, teaching and supervision.
The DMC RN:
• Establishes and maintains collaborative relationships with physicians and other health care providers
• Delegates, assesses, provides and evaluates patient care
• Provides and delegates patient care activities to team members
• Monitors patient progress and prepares patient for discharge
• Reports directly to senior unit management and participates in shared decision-making activities
MINIMUM QUALIFICATIONS
1. Graduation from a school of nursing required.
2. BSN preferred.
3. Licensed to practice as a RegisteredNurse (RN) in the state of Michigan required.
4. American Heart Association (AHA) BLS required.

DMC Rehabilitation Institute of Michigan is one of the nation's largest hospitals specializing in rehabilitation medicine and research. RIM is known for its clinical expertise in spinal cord injury, brain injury, stroke, amputee, orthopedics and catastrophic injury care. The Institute houses the Center for Spinal Cord Injury Recovery and the Southeastern Michigan Traumatic Brain Injury System (SEMTBIS), one of only 16 federally designated model systems of care for brain injury care and research. RIM also operates 31 outpatient sites throughout southeast Michigan specializing in sports medicine and orthopedics.
