Our client is currently seeking an Adult Care Facility Complaint Investigator (Clinical) to join their team. This is a full-time, long-term contract position.
Responsibilities Include:
Conduct on-site and off-site complaint, incident, death, and questionable operations investigations at facilities as assigned.
Follow all NYSDOH-established protocols pertaining to the assigned investigation, including but not limited to:
Interviewing residents, staff, and witnesses
Conducting records reviews
Site inspection
Investigation closure process.
Document all investigation activities in the appropriate data system(s) following all documentation timeline criteria.
Participate in closures and emergency events as assigned.
Attend and complete all training, both in person and remotely, as required.
Obtain Surveyor Minimum Qualifications Test Certification within one year of employment
Qualifications:
Registered Professional Nurse (RN) licensed and registered in the State of New York with:
One (1) year of survey or investigation experience, or;
Three (3) years of professional nursing experience in a post-licensure professional nursing experience, with at least two (2) years of which must have been professional clinical nursing experience in a licensed healthcare facility. A bachelor's or master's degree in nursing can be substituted for one year of the required general nursing experience.
Licensed Practical Nurse (LPN) licensed and registered in the State of New York with two (2) years of survey or investigation experience.
Surveyor Minimum Qualifications Test Certification, preferred.
Ability to work both independently and as a part of a team.
Ability to think critically, incorporating multiple factors into larger concepts.

We are in search of experienced clinicians to join a UAS Home Care Project. In this role, you will be responsible for independent review and evaluation of completed UAS reassessments for appropriateness of long-term care services for consumers, in accordance with Medicare and Medicaid guidelines.
These are contractor positions - which allow for flexibility in scheduling, at your own pace training, and pay at $100/hr.
In this role, you will be scheduled for in person appointments with members of the long term care population, who are applying for Managed Long Term Care services (MLTC). Your clients will have already completed a UAS evaluation with a RegisteredNurse, and your role will be to determine whether MLTC and home care services are adequate for the patients care, or if they should instead be admitted to a supportive facility. Each appointment will last approximately 1.5 hours - 30 minutes of preparation time, 30 minutes meeting with the member, and 30 minutes completing and submitting your determination. Additional compensation is offered for longer appointments.
Schedules are arranged according to your availability; however hours are not guaranteed. You must be willing to work at least 9 hours weekly, within operation hours which run Monday - Friday 8AM - 7PM, and Saturday between 10AM-6PM. Candidates who are unable to work during these operation hours, or who cannot commit to 9 hours weekly will not be considered.
Expect to travel! Appointments will be scheduled anywhere within the county or counties of your choice, based on your availability. Candidates covering multiple counties will receive more hours. Please note, you must choose one full county to be considered! Current openings are in:

How you will contribute
You will determine donor eligibility; to include, proper management of informed consent, AIDS education and confidential self-exclusion, medical history interview and acceptance requirements, physical examination, blood testing, and reviewing Serological Protein Electrophoresis (SPE).
You will evaluate donor reaction(s), which occurs at the facility as outlined in the SOPs. Follow applicable SOPs for medical emergencies including the development of Center Physician's standing orders and donor transport to emergency care facilities.
You will refer to the Center Physician or Medical Affairs (as applicable) when in need of providing unacceptable findings to donors, or guidance concerning medical or technical issues, including donor safety and eligibility.
You will support the Hepatitis B and Seasonal Flu vaccination programs for employees as applicable.
You will manage employee incidents and determine whether further evaluation is required by occupational health/ER. Refer to EHS guidance regarding employee incidents.
You can be a Pandemic Coordinator when authorized by EHS and support investigations associated with pandemic threats within the local community as indicated by EHS or Medical Affairs.

MINIMUM QUALIFICATIONS
Current NYS license to practice as a RegisteredNurse.
Driver's License that meets agency policy.
CPR certification.
Physical agility and ability to react to emergency situations, including Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines.
Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.

Responsibilities
Delivers safe care to assigned patients, while developing technical competencies
Recognizes overt abnormal signs and symptoms and/or changes in patients' condition and responds appropriately
Completes initial assessment of patient condition and response to treatment and initiates follow-up as needed

Uses the Computerized Patient Record System (CPRS) and the Bar Coded Medication Administration (BCMA) software per policy
Ensures that all JHACO standards for professional nursing care are met during performance of care
Completes appropriate medical record forms in compliance with VA guidelines and policies on each patient
Verifies the content and correctness of all recorded entries in patient record; all documentation/records/reports must meet or exceed established VA standards to include, but not limited to: timeliness, legibility, accuracy, content, and signature; outpatient records shall be completed the same day the patient is seen
Demonstrates sensitivity to cultural diversity, race, gender, and other individual differences in the workforce, in the Veteran population, and in the community

Qualifications
Holds a diploma, Associate Degree or Bachelor of Science in Nursing from an accredited program, college, or university and hold a current license to practice as a RegisteredNurse in any State; a Bachelor's degree is preferred
Has been practicing actively as an RN in an acute or long-term setting for at least the last two years
Is capable of providing complex and comprehensive nursing care for adult medical and surgical patients
Has and maintains current Basic Life Support (BLS) certification
Has no health or physical disability restrictions that interfere with performance of assigned duties
