Overview:
The RN Care Manager II - IPCM, as part of the interdisciplinary team, assesses, plans, advocates, and coordinates care from admission to discharge ensuring a safe transition post-hospitalization. This role involves assessing patients' health needs, developing individualized care plans, and coordinating services to optimize health outcomes. Effective communication with patients, families, and healthcare providers is essential in this role. Additionally, this is a key role in advocating for patients' needs, promoting health education, and facilitating smooth transitions of care across healthcare settings. Clinical expertise, critical thinking skills, and compassion are required and instrumental in promoting the well-being of patients and improving healthcare outcomes. Qualifications:
Education: Bachelor's Degree of Science in Nursing, required

Overview:
Mercy Women & Children's is a comprehensive service line that includes 12 Labor & Delivery Suites, 2 Obstetric Operating Suites, 6 High Risk Perinatal Suites, 14 Mother/Baby Couplet Care Suites, a Level 3 Neonatal Intensive Care Unit (NICU) with 19 private suites and special suites for multiples and a 20-bed Pediatric/GYN/Surgical unit.
The Transition Nursery RN is responsible for providing comprehensive care to newborns during the critical first two hours of life. This role involves attending deliveries, performing resuscitation, conducting thorough assessments, and ensuring the overall well-being of newborns. You will administer medications and treatments as prescribed and monitor and document newborns' progress and condition. Additionally, the RN will prepare women for cesarean sections (CS) and manage the recovery process for both vaginal and CS deliveries. The RN will also support the department by stocking supplies and performing various tasks as needed. The ability to communicate effectively with the healthcare team, including obstetricians, pediatricians, and other nurses.
Qualifications:
Education: Graduated from an accredited school of nursing

ESSENTIAL DUTIES
Regulatory: Coordinates completion of the student health record and maintains a cumulative health record for each student per the system used by the school district. Provides ongoing nursing support for students with identified conditions that necessitate consistent monitoring. Organizes and conducts student health appraisals to include medical examination, vision and hearing screenings, and height/weight measurements meeting state regulations. Coordinates the care for students who are injured or who become ill at school. Assists in providing emergency care for accidents and sudden illness of students until parents assume responsibility. Establishes communication, obtains proper consent and uses referral process for students to use the school-based health clinic or tele-med services when applicable.

Prepares stations for examination of patients; assures all areas are clean. Provides a positive impression, through words and tone, to students, parents, staff and visitors presenting to the clinic.
School-Based Clinic Patient Care and Discharge: Supports clinic providers in patient care to include specialty procedures/tests and providing injections, treatments and vaccines as needed. Performs phlebotomy and routine in-house laboratory tests as ordered. Ensure patients are educated about medications, provider orders, follow-up and other education as appropriate.
Other School-Based Clinical Duties: Maintains supplies for clinic and school nurses office observing district policies for ordering supplies; cleaning and sterilizing of instruments; transporting specimens, equipment and supplies utilizing appropriate and safe techniques.

In addition to performing visits and completing coordination of client care, the RegisteredNurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The RegisteredNurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
Qualifications
RegisteredNurses (RNs) must meet the following requirements:
Be currently licensed as a RegisteredNurse (RN) in the state of employment
A minimum of one year of clinical experience is preferred
Demonstrate knowledge and skill in current nursing practice
Possess a valid state driver's license and automobile liability insurance
