PRINCIPAL RESPONSIBILITIES AND DUTIES
CLINIC OPERATIONS:
Manages the operations of the clinic, including costs, processes, staffing, and quality standards.

Ensure education of the patient regarding quality measures, transplant options, modality awareness, and access care.
Acts as a resource for the patient and family to address concerns and questions.
Accountable for timely completion of patient care assessments and care plans.
Manages timely patient schedules to ensure facility efficiency and develops action plans for missed treatments.
Plans, coordinates, and validates patient eligibility for treatment.
Reports changes in patient status regarding any clinical, insurance, or travel and transportation issues.
STAFF:
Participates in the selection and hiring of new team members and ensures completion of new hiring orientation and mandatory trainings.
Responsible for all patient care employees completing appropriate training courses and that licensures/certifications are current.
Provides support for all clinical staff members at regular intervals and encourages professional growth.
Maintains current knowledge regarding company benefits, policies, procedures, and processes.

PHYSICIANS:
Facilitates the application process for physician privileges and compliance with Fresenius Medical Care Medical Staff By-Laws.
Responsible for strong physician relationships and ensures regular and effective communication.
Participates in Governing Body, an interdisciplinary team for each region including MSWs, Dietitians, MDs, DOs etc. that governs policies.
EDUCATION AND REQUIRED CREDENTIALS:
Bachelor's Degree or an equivalent combination of education and experience.
Graduate of an accredited School of Nursing (RN).
Current appropriate state licensure.
EXPERIENCE AND SKILLS:
Required:
6+ years business operations experience in a healthcare facility.
12 months experience in clinical nursing.
6 months chronic or acute dialysis nursing experience.
Successfully pass the Ishihara Color Blind Test.
Preferred but not required:

Works with the Manager of Post-Acute Services Care Networks, Care Management Manager, Social Work and other members of the CM team to identify patients requiring follow up assessments and identification of criteria for patients requiring follow up phone calls or other contact.

Summary:

Participates in the implementation of the unit specific Quality and Performance Improvement Plan, completes monitoring forms and reports findings to the Clinical Unit Supervisor and department leader.
Supports own professional development by participating in committees, cross-training to various roles, and precepting new graduates, transition of practice, and new hires.
Responsible for ensuring department is ready for patient care at all times, including but not limited to preparing patient rooms, maintaining cleanliness, and maintaining department survey ready.
Provide basic hygiene and ADL's to patients.
The nurse will exhibit flexibility in transfer of location throughout the shift as patient census dictates.

Conduct home visits to assess the patient's home environment per policy; initially and minimally annually thereafter. Routinely evaluate patient performance and assess the home environment more frequently as needed to improve care.
Communicate results of patient assessment, reassessment, and ongoing monitoring to the physician, team members, and others as appropriate to the individual needs of the patient.
Document all nursing services in the Electronic Medical Record including but not limited to training sessions, routine and non-routine in-person interactions, and phone conversations. Documentation should accurately reflect the patient status and nursing interventions and be written to ensure continuity of care.
Participate in infection control monitoring, implementation, and recording as requested.
Be familiar with emergency equipment and all emergency operational procedures. Communicate and regularly review Emergency Preparedness procedures with all home patients, including but not limited to emergency disconnection from dialysis equipment, what to do and who to contact if displaced from home.
Ensure clinic and patient supply inventory and usage is appropriate for patient prescriptions and place orders in timely manner to avoid fees and penalties.
Continually seek to improve patient and quality outcomes through tracking and trending of program and patient performance, use of critical thinking skills and root cause analysis to improve patient and quality outcomes. Assist in obtaining data for the continuous quality improvement activities.
Obtain routine and non-routine laboratory tests as ordered and communicate critical lab values and urgent patient needs to the responsible physician or physician extender in a timely manner.
Complete and document monthly review of patient medication profiles. Administer medications as ordered by the physician.
Responsible for performing on call nursing services, nights and weekends, on a rotational basis as needed or assigned.
Flexible with staffing locations and hours to accommodate patient and USRC home program needs.
Regular and reliable attendance is required for the position.
Home Hemodialysis Only:
Coordinate home evaluation with technical services department to ensure that the necessary electrical, plumbing and drainage requirements for proper equipment operation are met prior to patient acceptance into home program.
Ensure required and appropriate water sampling is complete per policy and product requirements. Monitor and report water sample and culture results.
QUALIFICATIONS/REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Requirements include:
Current RN license in applicable state. License must be maintained as current and in good standing.
12 months of clinical experience in providing nursing care and 3 months of experience in the specific modality for which the nurse will provide home dialysis training, except in states with a more stringent requirement such as Texas, which requires at least 6 months experience in the specific home modality.
Previous experience in providing nursing care to patients on maintenance dialysis preferred.
CPR certification required prior to patient care assignment.

Summary:

As the leader of the health care team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. Nursing Process:
Develops and/or contributes to the individualized plan of care that reflects assessment, planning, implementing, and evaluating the outcomes of that plan.
Ensures plan shows multidisciplinary planning, consultation, and education.
Ensures plan is reflective of admission or outpatient database, on-going findings, age appropriate care, cultural specific needs, and appropriate acuity.
Ensures plan is discussed with patient, family/significant others, and completed in a timely manner.
Monitors the patients progress based on the plan.
Revises plan on ongoing basis based on patient condition and evaluation of progress.
Ensures care meets standards of practice.
Ensures effective development and completion of discharge plan including discharge barriers and patient/family education. Ensures that patient clearly understands discharge instructions. In outpatient, identify barriers and needs for patient/family education that will facilitate the outpatient medical management plan.
Ensures patient safety related but not limited to, medications and procedures utilizing the five rights; patient falls; decubitus prevention and prevention of nosocomial infections.
Ensures optimal pain control and patient comfort; identifies and discusses patient anxieties, fears or concerns regarding patient condition, treatment or discharge.
Ensures that patient understands medication purpose, side effects, and administration instructions in the hospital as well as at the time of discharge .
Documentation:
Charting is accurate, legible, dated, and timed.
Documentation reflects nursing process and interventions and evaluations taken.
Utilizes computer systems effectively and efficiently for optimal patient care.
Clinical Outcomes:
Discusses patient findings and progress toward outcomes with physicians and other members of the health care team.
Demonstrates competencies during the probationary period and ongoing completion by departmental competency validation.
Make comprehensive nursing decisions based on interpretation of data, assessments, and evaluations of patient outcomes.
Participates in departmental performance improvement activities, i.e., planning, measuring/monitoring, assessing, and improving.
Workplace Safety:
Adherence to LMP Workplace Safety principles and practices.
Applies standard precautions; maintains a safe environment for self and others
Patient Care Experience:
Practices customer service standards as defined by the Service Area, Medical Center, and specified department.

Utilizes payroll and non-payroll resources to their maximum potential.
Basic Qualifications:
Experience
1-year recent (within the last 3 years) full-time equivalent experience in a Critical Care, DOU or SDU setting, or successful completion of a KP Critical Care, DOU or SDU course (as unit appropriate) or approved equivalent within prior 12 months.

Basic Arrhythmia course required

If twelve (12) hour option chosen, shift will be 7:30 pm-8:00 am, back up 8-hour shift will be 11:30 pm to 8:00 am.
