The Aftercare Case Manager provides ongoing support and coordination for youth and families as they transition from residential treatment to community life. This role ensures continuity of services, connects families to needed resources, and helps youth continue their healing journey through structured aftercare planning. The Case Manager collaborates with the Service Planning team, assists with school enrollment, medical and mental health provider transitions, and tracks aftercare outcomes to promote long-term stability and success. This role is based out of Abilene.
Key Responsibilities
Collaborate with the Service Planning team to create and monitor individualized aftercare plans.
Manage a caseload of approximately 25 youth receiving aftercare services.
Conduct and participate in aftercare check-ins to assess progress and needs.
Assist with school enrollment and maintain educational portfolios.
Coordinate medical and mental health service transitions to ensure no lapse in care.
Document all services and outcomes accurately and timely in accordance with agency and state standards.
Support parents and caregivers in accessing community resources and normative, age-appropriate activities.
Adhere to all New Horizons policies, state, and federal child care regulations.
Maintain a commitment to New Horizons' philosophy of Keeping Kids First in all actions and decisions.
Qualifications
Bachelor's degree and two years of experience working with youth who have experienced trauma preferred; or a master's degree in a social service or related field.
Valid driver's license, proof of insurance, and ability to transport youth as needed.
Strong organizational, communication, and documentation skills.
Ability to build positive relationships and work collaboratively across disciplines.
Understanding cultural diversity and the ability to work effectively with diverse populations.
Must pass a criminal background check, drug screen, and tuberculosis screening.
Base Salary Range: $38,000.00

ESSENTIAL JOB FUNCTIONS
Provide client access to CSBG, CEAP, TVC, TBRA and Weatherization Programs.
Conduct client eligibility and maintain appropriate documents.
Advise clients of their responsibilities of all programs.
Provide information to local citizens concerning all RPMC programs/activities/opportunities.
Enter data into the automated system in a timely and accurate manner.
Communicates all appropriate information to client on programs, requirements, and processes in a timely manner. Appropriately document contacts with clients in system.
Provide individualized budget consultations to clients, ensuring financial plans align with program goals.
Maintain an active caseload to meet the TOP performance outcomes
Maintain detailed and up-to-date documentation of client interactions, progress and outcomes daily for accurate monthly reporting
Conduct monthly outreach and collaboration with community partners to support program goals and expand client resources
Coordinate and ensure attendance at local job fairs to promote client opportunities and increase visibility of program services
Attend CSBG meetings bi-weekly
Coordinates with utility vendors on behalf of clients.
Maintains information on all community resources/activities/opportunities
Maintain knowledge of current eligibility guidelines, funding guidelines, and departmental policies; apply guidelines and policies to individual cases.
Prepare timely and accurate vouchers for payment
Responsible for program compliance. Ensure rules and regulations are followed. Ensure compliance with program goals, case management agreements, and policies and procedures.
Assure accurate and appropriate client records/documents are developed and maintained.
Responsible for ensuring a productive work environment.
Provide excellent customer service.
Provide case management services to eligible clients.

Qualifications
Bachelor degree with a major in case management or degree in closely related field; OR a non-related bachelor's degree or associate degree with one year case management/customer service work/social service work; OR a high school diploma/GED with five years case management/customer service/social service work experience.
