The Case Manager will inform the client(s) of the different programs and levels of care. The Case Manager will develop and implement strategies to ensure that client(s) transfer from each level of care (e.g., from Detox into Residential, from Residential to Outpatient/Housing, from Outpatient with Housing to Sober Living). The Case Manager will follow-up with client(s) to ensure that the client(s) whereabouts are tracked throughout their treatment stay. The Case Manager will follow-up with client(s) to ensure that they are enrolled into OHL's Community Health Center and adhere to their follow-up appointments. In coordination with Campus Health leadership, this position develops, implements, and enforces nursing standards, policies, and procedures utilizing the Standards of Practice for Nurses from the American College Health Association and Accreditation Association of Ambulatory Health Care. The Nurse Manager will institute a comprehensive training program to ensure that all clinical staff are knowledgeable regarding the performance of their duties and ensure optimum delivery of quality nursing practices and coordination of care. This position will direct efforts for infection control and safety and coordinate the success of these efforts with departmental and campus stakeholders. This is a full-time, 12-month position with some weekend and evening hours required. This position also may require participation in off-campus programs. * Skills in providing exemplary patient care and customer service. * Ability to maintain confidentiality and being reliably available to patients when they seek care or advice. * Knowledge and clinical background with public health or health education experience. * Demonstrated ability in all care to ensure compliance with regulatory and legal requirements. * Ability to maintain professional affiliations, licenses, certifications, etc., and stay current on developments in the field of medicine and college health. * Bachelor of Science in Nursing * Current nursing license to practice as a Registered Nurse in the State of Louisiana * 10 years of ambulatory or clinical nursing care experience * 3 years of infection control experience * 3 years of supervisory/managerial experience * 5 years of progressive experience in professional nursing management and administration * Possess the ability to assess, plan, develop, implement, and evaluate a clinical program * Experience with third-party billing and insurance * Five years of college health experience * Master of Science in Nursing degree
