Essential Duties & Responsibilities:
Performs client admissions, resumption of care, follow-up using OASIS data collection in a timely, legible, accurate manner.
Responsible for monitoring and managing core measurements pertinent to the department to assure compliance with the operating plan.
Identifies, documents and reports signs and symptoms of altered health status to physician and Clinical Team Manager. Obtains appropriate orders to increase/decrease visit frequency. Obtains medical orders to alter treatment plan.
Develops Plan of Care consistent with clinical assessment findings, diagnosis, orders, HHRG Case Mix category, and appropriate utilization guidelines/clinical pathway specific to each client. Sets and evaluates attainable, specific, and measurable goals and outcomes.
Administers meds and treatment according to Plan of Treatment.
Teaches nursing care appropriate to client's condition. Teaching based on client/caregiver level of understanding and is properly reflected in records.
Coordinates care, integrating other health team members. Participates in intra - disciplinary conferences as required.
Documents client records according to agency policy. Completes clinical notes, Plan of Care and medication records in a timely, legible and accurate manner.
Notes progress towards discharge planning. Communicates with physician, and appropriate staff regarding discharge planning.
Completes all discharge summaries according to policy and procedures.
Supervises the LPN and Home Health Aide to ascertain compliance with Plan of Care.
Makes supervisory visits in home at least every 14 days and documents visit in the clinical record to ascertain quality of care rendered.

The Manager monitors and evaluates the provision of nursing care, holds staff accountable for clinical outcomes, and drives operational efficiency in support of facility objectives, while leading staff to sustain an unparalleled patient experience.
Job Responsibilities
Manage and monitor nursing practice and overall unit operations, ensuring that patient and family needs are assessed and met in accordance with established policies.
Oversee staff scheduling and maintain optimal unit staffing levels in accordance with patient care needs and established productivity guidelines.
Drive continuous quality improvement by operationalizing evidence-based practices to enhance patient safety and quality outcomes.
Ensure strict compliance with all facility policies, procedures, regulatory standards, and internal/external benchmarks and accreditations.
Manage operating expenses, including supply and equipment utilization, and actively identify and implement cost reduction opportunities.
Facilitate effective interdisciplinary care coordination within and across departments to ensure efficient patient throughput and high-quality care.
Lead and inspire staff commitment to the organization's vision/mission/values, instilling accountability for providing consistent, positive patient interactions.
Requirements
Education
Graduation from an accredited School of Nursing is required.
Bachelor of Science in Nursing (BSN) is preferred (will accept candidates with an Associate Degree in Nursing (ADN) who are currently enrolled in a BSN program).
Experience
Recent (within the last year) Acute Care leadership experience in Critical Care is mandatory.
Specific experience requirements: Manager (2+ years), Supervisor (3+ years), or Charge RN (5+ years).
Note: Relief charge experience does not meet this qualification.
Certifications/Licenses
Current Florida Registered Nurse (RN) licensure is required.
Current Basic Life Support (BLS) certification is required upon hire.
Current Advanced Cardiac Life Support (ACLS) certification is required upon hire.
Skills
Proven ability to manage a unit budget and identify opportunities for cost reduction and efficiency improvements.
Expertise in operationalizing evidence-based practices and ensuring compliance with regulatory and accreditation standards.

Oversees a team of 50 direct reports (Scrub Techs, Coordinators, Surg Techs, OR Aides, RNs, etc.)
QUALIFICATIONS
Associates Degree in Nursing required; Bachelors in Nursing preferred.
Current FL RN license required
Previous experience in an acute care setting strongly preferred
BLS and ACLS
