Initiates diagnostic and palliative nursing procedures as appropriate to the patient's care and safety. Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so.
Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient's condition.
Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient's condition. Utilizes agency educational material as appropriate.
Develops, prepares and maintains individualized patient care visit records that reflect the patient's plan of care with accuracy, timeliness, and according to policies. Submits accurate documentation within 24 hours of visit.
Responsible for following all policies and procedures of the agency regarding service delivery documentation and care coordination. During supervisory visits exhibits full compliance and verbalizes knowledge of agency policies including, but not limited to, patient education, infection control, management of hazardous wastes and age-specific behaviors for patients.
QUALIFICATIONS
Valid Florida RN license.
Minimum one year of clinical experience. Hospice experience a plus!
Valid Florida driver's license and required auto liability insurance.

Responsibilities:
The RegisteredNurseCaseManager will practice within the scope of his/her licensure.

Assess problems and determine goals and actions designed to meet the needs of the patient and document into the case notes. Be aware of any alternative treatment possibilities that may allow the patient to reach wellness goal(s). If there are no benefits available for your recommended alternative treatments, provide to the payer a cost-benefit analysis to demonstrate that extra-contractual services will enhance the patient's medical condition and will be cost-effective to the benefit plan.
Become familiar with community resources and funding sources so that the patient can receive quality health care and conserve health benefit dollars. Many agencies exist which provide assistance to persons in financial need or to provide information to persons with specific medical conditions.
Maintain case in computer system documenting case actions for each patient under your casemanagement. Complete all aspects of case in the computer. Prepare timely reports to the payer to detail all case actions, the results of those actions, and the continuing casemanagement plan.
Maintain billing as appropriate in computer system.
Continue to maintain contact with the providers and with the patient across the continuum of care to be sure that patient needs are being met. On any cases which include a chronic condition keep the file open for periodic contacts to verify the clinical status of the patient and additional medical needs.
Negotiate with providers to maximize the medical benefits available to the patient. Make network referral as appropriate.
Act upon any awareness of non-medical issues which involve the patient's safety or welfare. Attempt to direct the patient or family to appropriate providers or community resources, or to personally notify appropriate authorities. Consult with the CM supervisor on a regular basis, and keep the supervisor informed regarding any complaints which may occur about casemanagement services or any issues which arise which the casemanager is not competent to handle or does not have the expertise to handle.

All casemanagers will possess a URAC-recognized certification in CaseManagement within 3 years of hire. The salary range for this position is from $72,000 to $81,000 annually.

Minimum Qualifications:

Ensures a current physician order, plan of care/ care plan is place for all assigned patients
Manages the patient care by supervision and care coordination for all assigned patients and including the disciplinary team
Ensures that the competency program is followed and provides educational support for the caregivers as needed for skills development
Promotes employee engagement and retention
Participates in the review of policies and procedures to ensure compliance with all State, Federal, and Accreditation standards
Participates in the quality assurance performance improvement (QAPI) program
Assists in the coordination and oversight of all plans of correction and quality initiatives as directed by the DON
Partners with the DON and DOO to lead cohesive teams toward common goals
Assists management with the development and implementation of new products and services
Maintains professional, supportive, and responsive interpersonal communication skills

ROLE QUALIFICATIONS
EDUCATION & EXPERIENCE REQUIRED
RN Nursing Degree. Unencumbered RN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact where recognized
RegisteredNurse with 1 year of clinical experience
Valid driver license with proof of insurance
Working knowledge of home care regulatory and federal requirements
Proven customer service skills, sound independent judgment, initiative and solid organization and time management skills required
Strong computer skills to include Microsoft Office Suite
Ability to travel to Family First office locations and patient homes throughout designated area
