The Role Itself

What You'll Do
Strive's RN Care Managers are a critical part of supporting patient care for individuals with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). Serving as a vital resource for patients, families, local care teams, and healthcare professionals, the RN Care Manager coordinates services to optimize care delivery. While primarily conducted via telecommunication, this role may necessitate on-site visits to various healthcare settings or patient homes. The Day to Day
Works closely with the care team to craft personalized and thorough care plans addressing the clinical and non-clinical needs of each patient.
Conducts regular assessments onsite and virtually to detect undisclosed medical or social issues and changes in the home environment that could pose risks, promptly referring such matters to the appropriate channels for resolution.
Initiates proactive outreach to patients and/or clinics as outlined in program guidelines to evaluate their status, offer assistance, and address inquiries promptly.

· Outcomes management and evaluation of the achievement of established goals within the
plan of care.
· Ensures initial and annual follow up Model of Care training occurs for employees, providers
and contracted personnel associated with ISNP program
Qualifications:
· Current unrestricted RN license for the state in which the Nurse Care Coordinator is
practicing.
· Minimum of 3 years clinical care experience.
· Skilled Nursing Facility/Long Term Care experience preferred.
· Case management experience preferred
· Strong nursing expertise in managing complex cases with minimal supervision.
· Knowledge of current standard medical practices and experience with
Medicare/Medicaid.
· Ability to work with and communicate on a professional level with the interdisciplinary
team.
· Excellent independent critical thinking/problem-solving skills.
· Excellent oral and written interpersonal communication, organizational, multi -tasking,
and teamwork skills.
· Proficiency in nursing/medical databases and Microsoft programs.
· Positive and enthusiastic attitude.
· Valid driver's license and car insurance with reliable transportation.

Key Responsibilities:
• Perform nursing assessments, ensuring clinical competencies are met.
• Evaluate client/patient physical conditions and address service needs.
• Coordinate care plans based on collaboration with treatment teams.
• Provide health education and engage clients in disease prevention strategies.
• Administer and oversee medication requirements, ensuring compliance.

Education, Experience, and/or Credential Qualifications:
• Current Registered Nurse licensed in the state of practice.
• Must be willing to obtain licensure in additional states.
• Comfortable providing telehealth services.
• FQHC: BCLS certification required; ACLS certification preferred.
Additional Qualifications:
• Successful completion of a background check, including criminal record and driving record.
• Current driver's license, acceptable driving record, and current auto insurance.
• Completion of New Hire Orientation and annual training requirements, including Relias training.
Physical Requirements:
• Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects.
• Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
