Summary:
A Stepdown RegisteredNurse provides care to patients who are transitioning from intensive care to medical-surgical or general inpatient units. These patients require frequent monitoring and specialized nursing interventions. They assess and manage complex patient needs, administer medications and treatments, and collaborate with multidisciplinary teams to ensure safe, patient-centered care.
Essential Work Functions:
Monitors and interprets cardiac rhythms, vital signs, and other telemetry data to detect abnormalities and respond appropriately
Administers prescribed medications and treatments in accordance with approved nursing techniques, with a focus on cardiac care
Prepares equipment and aids physician during cardiac-related procedures and examinations
Maintains awareness of comfort and safety needs of telemetry patients
Observes patient, records significant conditions and reactions, and notifies supervisor or physician of patient's cardiac status and response to interventions
Responds to life-saving situations based upon nursing standards, policies, procedures, and protocols specific to cardiac emergencies
Documents nursing history and physical assessment for assigned telemetry patients
Initiates patient education plans according to individualized needs, focusing on cardiac health, risk factors, and lifestyle modifications
Collaborates with the interdisciplinary team to ensure comprehensive care for telemetry patients
Maintains confidentiality of patients and client
Performs other duties as assigned within the scope of practice
Adheres to hospital safety protocols, infection control guidelines, and regulatory standards

• Assesses the physical, social, psychological, living conditions and pastoral care needs of the patient/family through professional knowledge and skills of observation, interviewing and physical examination on an ongoing basis.
• Establishes and revises, as necessary, the nursing component of the patient/family plan of care based on goals mutually acceptable to the patient/family and significant others and makes referrals to other services, as necessary.
• Coordinates implements and evaluates the effectiveness and outcome of the care plan by ongoing assessment, planning, coordination, counseling, teaching and supervision.
• Travels to patients' homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
• Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 - 30 lbs. to and from patient homes/care facilities, VNS Health offices and other locations.
• Provides nursing care in accordance with Agency policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care.
• Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals.
• Visits clients every two weeks to assess the clients' condition and update the plan of treatment.
• Monitors the effect of the plan of cares on the patient/family and informs physician and other personnel of changes in the patient's needs.
• Manages a caseload of patients.
• Provides nursing care services and initiates appropriate preventive and rehabilitative nursing procedures.
• Participates in interdisciplinary team meetings.
• Appraises care provided and revises the goals and approaches to patient/family care in coordination with the interdisciplinary team.
• In coordination with the interdisciplinary team, assesses the family's needs during the bereavement period and provides ongoing counseling with referral to community resources, as appropriate.
• Initiates and maintains written (including clinical and progress notes) and verbal communications to assure optimal patient care.
• Understands and interprets Hospice Care reimbursement policy and determines the billing source for patient care.
• Maintains productivity sufficient to meet Hospice goals.
• Participates in the orientation of new staff.
• Orients, instructs and supervises License Practical Nurses, paraprofessional staff and volunteers.
• Consults with and educates the client and family regarding the disease process, self-care techniques, and prevention strategies.
• Performs established work procedures in relation to Region/Program workflow.

Work Experience:

RESPONSIBILITIES:
Direct day-to-day nursing operations on the unit under the oversight of the DON.
Supervise, delegate, and coordinate care provided by assigned nursing staff (RNs, LPNs, CNAs).
Perform regular rounds to assess residents' physical and emotional status; make daily visits to monitor and evaluate condition.
Provide direct nursing care as needed, including starting IVs, collecting lab specimens (e.g., sputum, urine), and other treatments as ordered.
Admit, transfer, and discharge residents as necessary, coordinate smoothly with interdisciplinary team.
Maintain accurate, timely documentation of residents' medical status and any changes - including physician's orders, lab results, and care interventions.
Communicate proactively with residents' families or POAs and with attending physicians regarding status updates and changes in condition.
Oversee completion of assessments (e.g., MDS) and ensure reports and resident care plans are completed in a timely fashion.
QUALIFICATIONS:
Valid, unrestricted RN license in Connecticut.
Graduate of an accredited college nursing program.
Current CPR/BLS (or equivalent) certification as required by facility policy.
Strong interpersonal and communication skills; ability to work well with residents, families, and the care team.
Compassionate, resident-centered approach, with good organizational and time-management skills.
Comfortable working in a long-term care or skilled nursing environment (experience preferred).

Summary:
The Travel Cath Lab RN provides specialized care for patients undergoing cardiac catheterization and interventional treatments. The RN prepares patients for procedures, assists during interventions, and monitors immediate post-procedure recovery. Working closely with cardiologists and cath lab teams, this role requires advanced cardiac care skills, cardiac catheterization skills, strong communication, and the ability to adapt quickly while maintaining strict safety and regulatory standards.
Essential Work Functions:
Conduct assessment of patients prior to procedures, including reviewing medical history and performing physical exams
Assist in positioning patients correctly for the procedure and ensure EKG electrodes and cardiac leads are properly placed
Assist physicians during procedures, including the placement of catheters, wires, balloons, stents, structural heart devices, and other cardiac interventions
Operate and monitor hemodynamic equipment, recognizing and responding to changes in cardiac rhythms and hemodynamic waveforms
Assist in arterial and/or venous sheath removal using closure devices, manual pressure, or mechanical compression, and transport patients to the appropriate recovery area
Monitor vital signs, ECG, and hemodynamic parameters during cardiac catheterization and interventional procedure
Support post-procedure care, including monitoring patient recovery and ensuring a smooth transition to the next phase of care
Maintain and troubleshoot cath lab equipment, ensuring everything is functioning properly and fully stocked for procedures
Communicate effectively with team members, patients, families, and physicians to provide seamless care
Adhere to hospital safety protocols, infection control guidelines, and regulatory standards

Job Summary

Responsibilities
Appropriately assesses and plans patient care by: accurately and reliably obtaining, assessing and interpreting data; identifying and documenting problems and expected outcomes based on a plan of care for the preoperative patient; utilizing scientific knowledge base and appropriate physiological/psychological/social/cultural/spiritual concepts; organizing care and interventions; and identifying appropriate resources in order to meet individual and family health goals.
Using evidence based recommendations, coordinates and implements patient-centered care with variances along the continuum by: contributing to and following plans of the health care team and partnering with patient/caregivers; ensuring documentation and communication amongst caregivers is complete; delegating and ensuring completion of activities in alignment with patient safety; and educating patients/caregivers to achieve established health goals in an effort to provide coordinated seamless care.
Utilizes data to identify actual/potential perioperative risk factors, preadmission services and special needs for initiating referrals and implementing additional interventions as indicated. Collaborates with other members of the health care team on pertinent patient care concerns that directly impact admission.
