PURPOSE AND SCOPE:
The professional registerednurse Outpatient RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations.
Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other RegisteredNurses.
Performs ongoing, systematic collection and analysis of patient data pre - during - post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed.
Assesses, collaborates, and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.
Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients.
Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness.
Initiates or assists with emergency response measures.
Serves as a resource, leader, coach, mentor and role model for new and incumbent employees by setting examples of appropriate behavior, work habits and attitudes towards patients, co-workers, supervisors and the company at the facility, and area level.
Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
Identifies expected outcomes, documents, and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.
Ensures patient awareness related to transplant and treatment modality options.
Participates in education and quality improvement projects at the facility and area level as directed by Supervisor.
May serve as a Preceptor to new employees.
Required to complete CAP requirements to maintain or advance.
Performs all other duties as assigned by Supervisor.

SUPERVISION:

EDUCATION and LICENSURE:
Graduate of an accredited School of Nursing.
Current appropriate state licensure.
Current or successful completion of CPR BLS Certification
Must meet the practice requirements in the state in which he or she is employed.
EXPERIENCE AND REQUIRED SKILLS:
Entry level for RNs with minimum of 2 years of Nephrology Nursing experience
The rate of pay for this position will depend on the successful candidate's work location and qualifications, including relevant education, work experience, skills, and competencies.

Summary:

Essential Work Functions:

Assist in positioning patients correctly for the procedure and ensure EKG electrodes and cardiac leads are properly placed

Operate and monitor hemodynamic equipment, recognizing and responding to changes in cardiac rhythms and hemodynamic waveforms

Monitor vital signs, ECG, and hemodynamic parameters during cardiac catheterization and interventional procedure
Support post-procedure care, including monitoring patient recovery and ensuring a smooth transition to the next phase of care

SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants' needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 “on-call” process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.
Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant's health problems.
Performs the duties of Home Care Coordinator on the IDT as needed / assigned. May perform the duties of other IDT members based on professional licensing, competencies, and experience as needed.
Actively participates in utilization review meetings and quality improvement projects / meetings.
Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated.
Participates in family meetings, staff meetings, in-service and training and orientation programs as required.
Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families.
Practices standard precautions and follows PACE Program Infection Control protocols.
Performs other duties as required or requested.
EXPERIENCE, EDUCATION AND CERTIFICATIONS:
Bachelor of Science in Nursing Degree preferred.
State RN License required
**NJ: Licensed by the New Jersey State Board of Nursing.
BLS required (must have within 90 days of employment).
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
Experience in home care, long-term care and / or managed care preferred.

Bilingual with Fluency in Hmong required.

Job Specific Position Duties: The duties include, but are not limited to the following:
CORE Duties:
Participate in all phases of the CaseManagement Program (CMP) and ensure that the CMP meets established casemanagement (CM) standards of care.
Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.
Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM
Develop and implement tools to support casemanagement, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
Integrate CM and utilization management (UM) and integrating nursingcasemanagement with social work casemanagement.
Maintain liaison with appropriate community agencies and organizations.
Accurately collect and document patient care data.
Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.
Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
In addition to the duties listed in the basic contract, the following additional or supplemental duties are required for HCWs on this TO. The duties for the HCW are as follows but not limited to:
Maintain adherence to Joint Commission, URAC, CaseManagement Society of America (CMSA), and other regulatory requirements. Apply medical care criteria (e.g., InterQual).
Provide input on MTF CM resources and make recommendations to the Command as to how those resources can best be utilized.
Work in conjunction with the entire healthcare team and other departments, to identify high-risk and/or high-utilizer populations to include but not limited to those beneficiaries with multiple providers, multiple admissions/readmissions, Emergency Department visits, catastrophic illness, chronic or terminal illness, and multiple medical problems/dual diagnoses.
Provide casemanagement advice and consultation.
Collaborate with other members of the healthcare team, the patient and/family/support system on a regular basis to establish and update the casemanagement plan of care using evidenced-based guidelines (when available and/or applicable).
Identify measurable short-and long-term goals/outcomes of care with matching strategies to achieve optimal wellness and autonomy (self- management). Incorporate the patient's cultural background, values and beliefs, readiness to learn and healthcare needs across the continuum of care into the plan.
Provide the patient/ family with the knowledge and skills necessary for the implementation of the established plan. Facilitate patient and family decision-making activities by keeping them well informed of their rights, responsibilities and options. When indicated, follow patients through hospitalization and follows up in ambulatory and community health care settings.
Actively measure the patient's response to the evidence-based plan of care and provide documentation that the plan and the quality of the services offered to the patient correspond to the identified needs.
Ensure appropriate health care instruction to patient and/or caregivers based on identified learning needs.
Facilitate multidisciplinary discharge planning and other professional staff meetings as indicated for complex patient cases and develop a database and knowledge of local community resources.
Develop and implement mechanisms to evaluate the patient, family and provider satisfaction and use of resources and services in a quality-conscious, cost-effective manner.
Collaborate with the multidisciplinary team members to set patient-specific goals. Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
Close cases when goals are met, patient declines service, patient transitions to another casemanager or patient needs are no longer identified.
Provide casemanagement orientation and education for other casemanagers new to the role and/or facility providing scientific and practice-based knowledge per the CaseManager Core Competencies.
Provide support to other casemanagers, including managingcaseloads during absences.
Facilitate and coordinate strategies to ensure smooth transition and continued health care treatment for patients when the military member transfers out of the area. Develop a policy for, and assist with, region-to-region transfers. This shall include coordination of required tests, procedures, treatments, discharge planning, community referrals, and transfers.
Facilitate screening and assist with transfers of Exceptional Family Member Program (EFMP) families and service members going through the Integrated Disability and Evaluation System (IDES).
Plan for professional growth and development as related to the casemanager position and maintenance of CM certification. Actively participate in professional organizations including participation in at least one annual national CM conference to be funded by the government to be scheduled at the convenience of the government and the HCW. Ensure all training completion documentation is submitted to the region and HQ, as required.
Participate in video teleconferences (VTCs) and other meetings as required.
Provide safe, quality clinical casemanagement services to a variety of eligible beneficiaries, in accordance with the Department of Defense (DoD), BUMED, Regional, directorate, and departmental instructions, policies and procedures.
Keep informed of research and new information that will ensure new methods and practices are incorporated into the casemanagement program; attends continuing education programs, seminars, and conferences in order to maintain core competencies in casemanagement.
Facilitate command cost containment through proper utilization of available resources and timely assessment of patient response to the casemanagement program.
Assist in the design, implementation, sustainment and ongoing improvement of the casemanagement program.
Increase MTF staff involvement in and support of casemanagement initiatives by providing orientation and ongoing education and in-service training specific to casemanagement and the program.
Perform clinical assessments of the patients and managed care records that include clinical input from various health care providers across all clinical areas.
Evaluate and treat outpatients and inpatients using laboratory and clinical findings and differentiate between contributory causes.
Perform follow up clinical assessments to ensure the effectiveness of treatment plans in place.
Perform all charting on the computer, utilizing various programs.
Integrate casemanagement with utilization management and disease management as needed.
Minimum Qualifications:
* Education: BSN
* Experience: Minimum 1 year experience as a RN CaseManager within the last 3 years
* License: Active, unrestricted RN license
* Board Certification: Must possess a casemanagement related certification (CCM, CDMS, CRRN, COHN, COHNJ-S, ACCC, CRC, RN-NCM, or CMC)
* Life Support Certifications: BLS
* Security: Must be able to pass a Government background check and obtain a Government security clearance.

RegisteredNurse Care Manager
SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants' needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 “on-call” process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.
Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant's health problems.
Performs the duties of Home Care Coordinator on the IDT as needed / assigned. May perform the duties of other IDT members based on professional licensing, competencies, and experience as needed.
Actively participates in utilization review meetings and quality improvement projects / meetings.
Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated.
Participates in family meetings, staff meetings, in-service and training and orientation programs as required.
Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families.
Practices standard precautions and follows PACE Program Infection Control protocols.
Performs other duties as required or requested.
EXPERIENCE, EDUCATION AND CERTIFICATIONS:
Bachelor of Science in Nursing Degree preferred.
State RN License required
**NJ: Licensed by the New Jersey State Board of Nursing.
BLS required (must have within 90 days of employment).
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
Experience in home care, long-term care and / or managed care preferred.
1 year experience providing care as an RN required.
Bilingual with Fluency in Hmong required.
PRE-EMPLOYMENT REQUIREMENTS:
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
