Under the general supervision of the Supervising Registered Nurse RN IV and Center Director, the Registered Nurse II is responsible for providing clinical care and working to integrate whole person care into our new team model. The registered nurse will participate in telephonic and in person patient triage, education and counselling, participating in the ongoing education of the care team and collaborating with medical providers to ensure optimal health of our patient populations. Responsibilities * Coordinate team-based care by focusing on the intersection between medical and social needs of our patients. * Acts as a resource for support staff for triaging walk in and telephonic care. Job Requirements * Bachelor's Degree in Nursing. * Current and valid California Registered Nurse license and BLS. * Proficient in Microsoft Office Suite. * Electronic health records, Epic preferred.
