Job Summary for Registered Nurse Case Manager (RNCM)
Plan, organize and direct hospice care services, from initial evaluation and assessment, pain and medication management, and care plan development.
Ensure high quality patient experience and positive health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on patient needs.
Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team and collaborate with the patient's primary care physician to ensure continuity of care.
Provide coaching and education to patients and their caregivers based on clinical diagnosis, empowering them to self-manage their condition, prevent complications, and improve quality of life as a Registered Nurse Case Manager (RNCM)
Minimum Qualifications
Current CA Registered Nurse (RN) license
Current CPR card for Healthcare Providers
1 year of Registered Nursing experience required
1 year of Registered Nurse (RN) in hospice preferred
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
Self-directed and able to work with minimal supervision.
