Responsibilities
Conduct comprehensive clinical assessments (including medical history, risk factors, and medication review) and develop person-centered care plans with SMART goals based on medical, behavioral, and social needs.
Provide medication reconciliation, health education, and condition-specific teaching to strengthen member understanding, self-management, and adherence.
Collaborate with Lead Care Managers, CHWs, Behavioral Health, Housing Navigation, providers, and community partners to coordinate services and resolve medical and social barriers.
Triage clinical concerns, identify red flags, and escalate appropriately to NP/MD partners; provide brief interventions within RN scope as needed.
Participate in interdisciplinary Systematic Case Reviews (SCR), IDT meetings, and case conferences, offering clinical recommendations and follow-up planning.
Coordinate transitions of care (TOC) by supporting post-hospital follow-ups, reconciling medications, scheduling timely appointments, and ensuring continuity.
Maintain accurate, timely, audit-ready documentation in ECW, Google Suite, and payer/health plan portals; ensure all clinical assessments, screenings, and care plans meet required timelines.
Engage members through relationship-based and trauma-informed approaches, building trust with individuals who may struggle with traditional healthcare systems.
Identify gaps in care, clinical risk, or environmental barriers and collaborate with care teams to implement timely interventions.
Support HHN's startup model by adapting to evolving workflows, contributing to clinical process improvements, and helping build scalable care coordination practices.

Primary Duties

Position Summary
The Perinatal Nurse is a Registered Professional Nurse assigned to the Perinatal Services Unit, who is responsible for the direct delivery of patient care using the nursing process of assessment, planning, implementation, and evaluation, to a specific group of ante partum, laboring and delivering, recovering, and post partum mothers, as well as their newborns. The Perinatal Nurse coordinates care in collaboration with the family, significant others, physicians, and other health care providers. The Perinatal Nurse is trained and able to initiate, modify, or stop treatment when appropriate and according to established protocols, even when a physician is not present. Job Description

Coordinates and monitors patients through the hospitalization process utilizing physicians, primary care nurses, and third party payors to maintain quality care and fiscal responsibility.
• Assists the admitting department in identifying and resolving problems regarding appropriate admissions.
• Acts as a resource/advisor to physicians for adequate medical record documentation, appropriateness of facility services as they relate to diagnoses, and options for post-discharge care.
• Reviews the post hospital care plan with the patient/family. Establishes a contract regarding time frames and responsibilities. Follows plan through to discharge.
• Utilizes community services to provide optimal discharge planning, monitors/expedites timely completion of services to avoid delays in care or discharge, facilitate third party reimbursement approval and/or authorization, conducts post discharge activities and documentation, responsible for completion and interpretation of patient statistical reporting.
• Coordinates with multi-disciplines to include Dietary, Physical Therapy, Occupational Therapy, Speech, Social Services, Infection Control, etc., to insure efficient use of hospital resources.
• Answers questions from patients and their families, including providers regarding reimbursement of covered and non-covered services.
Hours for this Position:
Monday - Friday Day time shift - Every other weekend required

Qualifications
Graduate from an accredited RegisteredNursing Program or Licensed Vocational Nursing Program with current California license.
Two to five years clinical experience, managed care experience preferred.
