As the Executive Director, you will serve as the Administrator of the entire home health provider and is responsible for the oversight of the day-to-day operations. Job Summary
Odyssey House Louisiana, Inc. is seeking a full-time NurseManager for the Community Health Center. The schedule is Monday-Friday 8am-5pm. This role is a full-time grant funded position by the Hepatitis C Elimination Initiative Pilot. The NurseManager plays a critical role in staff development, clinical leadership, and fostering a supportive, high- performing care environment within the Hepatitis C (HCV) grant program. This position oversees and assists practitioners in coordinating the physical and behavioral health needs of individuals living with HCV and other related medical conditions, ensuring all services are delivered through a low-barrier, patient-centered approach. The NurseManager also works directly with program participants to identify and address challenges in accessing HCV treatment and medications, helping to remove barriers and guiding individuals through each step of their care journey.
Duties & Responsibilities
It is the duty and responsibility of the NurseManager to:
Schedule nursing shifts ensuring that all shifts are covered, and if needed work any open shifts
Review nurse's timecard activity for accuracy and ensure compliance with agency policies and procedures
Conduct nursing meetings
Supervise, orientate, and assist staff in delivery of patient care
Oversee clinical matters including but not limited to clinical documentation, and reporting of abuse and neglect
Assist CHC Medical Manager in supervision of Nursing staff through implementation and review of current policies and procedures, medication administration practices, infection control, direct care, staff with scheduling, in-servicing, training, and education
Assist patients and providers with adherence to evidence-based treatment protocols for specific disease states
Identifies at-risk populations using approved screening tool(s) and follows established care and reporting protocols
Use data to drive decisions and plan/implement performance improvement strategies related to care management for assigned patients, including fiscal, clinical, and patient satisfaction data
All infectious disease screening will be offered to program participants by the Odyssey House Community Health Center (OHCHC). Nurse will coordinate testing, treatment, and ongoing care within both OHCH locations
Ensure compliance with HIPAA Privacy Rules
Participate in weekly one on one meetings and monthly in-services
Work collaboratively with other departments including but not limited to Accounting, Human Resources, Development, and Compliance and Quality Assurance
Participate in clinical discussions
Complete incident reports and ensure nurses are completing Incident Report per agency policy, ensure all incident reports are submitted to Program Manager/Nurse Practitioner Supervisor promptly
Always maintain professionalism with staff members and clients
Coordinate/facilitate patient care, work collaboratively and maintain communication with patient care team (providers, counselors, etc.) to provide effective, timely, and appropriate patient management
Meet with Community Health Center
Perform other assigned duties
MINIMUM QUALIFICATIONS
Electronic Health Record experience

What is the purpose of this role?
The Nurse Care Manager (NCM) plays a vital role in fulfilling Nest Health's mission to make comprehensive healthcare radically accessible for families and children. By adopting a family centered approach, the NCM works to reduce unnecessary emergency room visits, coordinate care effectively, and prevent the worsening of chronic conditions. Through these efforts, the NCM helps empower families and communities to achieve their full potential. The Nurse Care Manager (NCM) reports to the Health Services leader and provides oversight, guidance and support for family care plans developed in collaboration with Nest Health clinicians and interdisciplinary team members. The NCM maintains a deep understanding of the patient panel and helps synthesize and prioritize family and patient needs and action plans based on family goals, acuity, quality gaps, and social determinants of health. In collaboration with other members of the healthcare team, the NCM is responsible for organizing, coordinating, and providing care coordination and care management services to patients and families.
What will you do?
Care Management & Coordination Responsibilities
Manage market panels to ensure patients and families are getting the right care at the right time through identification of high-risk patients, admission/discharge/transfer notifications, claims data, and findings from visits with Nest. Aggregate individual family members' care plans to understand and create a clear plan for the family. Lead interventions and coach Family Advocates on priority interventions.
Identify social determinants of health and work with interdisciplinary team to support family needs and decrease barriers to care.
Through various modalities (in-person, virtual, or by phone) and based on the patient's plan of care, the NCM will provide timely, relevant, and clear education to improve patient/family understanding of their conditions and how manage those conditions to improve health outcomes.
Act as clinical liaison for payer-based Care Management programs supporting both programmatic and individual patient needs as indicated.
Ensure accountability and follow up for patient care needs through panel management, case studies, and chart reviews. Support intradisciplinary team members on what they need to execute care plans and best support patients.
Use data and reporting to identify gaps in care or services and conduct patient outreach to facilitate follow-up care or services.
Visit, educate, and serve as a resource to providers on patients with chronic and/or high-risk conditions that could benefit from extra services to provide coordination and linkage to general medical service.
Communicate and coordinate with specialists, mental health and nutrition providers regarding Nest patients as needed to ensure patients are receiving the care they need both inside and out of Nest.
Quality Improvement & Organizational Commitment
Follow-up with patients within a reasonable amount of time that the information regarding inpatient discharge and ED visits information is made available to Nest Health.
Complete chart review and preparation ahead of visits specifically related to hospital utilization, social determinants of health, HEDIS quality gaps, and immunization needs.
Assist in provider efficiency by pending immunizations due for pediatric patients.
Provide feedback and insights on Nest's care model for continual refinement to best support patient care and employee experience.
Intermittently support field team on field visits to fully understand and appreciate the in-home care patients and families receive.
Collaborate to engage new patients and support outreach efforts.
Other duties as assigned.
What do you bring to the Nest?
Required: Louisiana RN unincumbered license required, compact license a plus
BSN preferred
Three years of clinical practice in a hospital, clinic, or home care with pediatric experience being highly desired
One year of experience in value-based care, population health, or care coordination preferred
