As a Home Health RegisteredNurse, you will:
Provide admission, case management, and follow-up skilled nursing visits for home health patients.
Administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
Coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.

Work Environment
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam, and Washington, D.C., to Washington state. As a Navy Nurse, you may work at one of the highly acclaimed National Naval Medical Centers in Bethesda, MD, Portsmouth, VA, or San Diego, CA. You might even provide medical support aboard one of two dedicated hospital ships-the USNS Comfort and USNS Mercy. Additional opportunities are available on surface ships, with aircraft squadrons, or even with the Fleet Marine Force.
Training & Advancement
Upon commissioning, Nurses who are new to the Navy are required to attend Officer Development School (ODS) in Newport, RI. ODS is a five-week program designed to introduce you to Navy culture. Nurses commissioned through a school ROTC program may not need to attend ODS.
Officer training is complete, you will learn the ins and outs of life as a Navy Nurse before receiving your first posting. Promotion opportunities are regularly available but competitive and based on performance.
The Navy provides Nurse Corps Officers the opportunity to specialize based on manning needs. Specialization fields include:
Nurse Anesthetists - Administer general and regional anesthesia, monitor patients receiving anesthesia, and assist in instructing medical trainees and other Officers
Primary Care Nurse Practitioners - Provide comprehensive health care and health maintenance for service members and their families
Medical-Surgical Nurses - Assess, plan and implement direct nursing care of patients on an assigned unit, and assume charge nurse responsibilities
Perioperative Nurses - Plan, implement and evaluate nursing care of surgery patients
Critical Care Nurses - Provide highly skilled, specialized nursing care to critical patients, including en route care, and train personnel in critical care nursing procedures
Mental Health Nurses and Nurse Practitioners - Provide direct patient care in mental health services, and lead and train other military and civilian personnel
Military-specific Specializations - Education and training, manpower systems analysis, and nursing research

Qualifications & Requirements
A degree from a four-year college or university is a minimum educational requirement to become a Commissioned Officer. You must also attend Officer Training. There may be exceptions to the degree requirements based on extensive service experience. Additional qualifications include:
U.S. Citizen between the ages of 18 and 41
Currently licensed and practicing nursing in the U.S. (new graduates must obtain a license within one year of beginning Active Duty service)
In good standing (as a student or graduate) with a CCNE-accredited U.S. education program granting a Bachelor of Science degree
Willing to serve a minimum of three years Active Duty
In good physical condition and able to pass a full medical examination
General qualifications may vary depending upon whether you're currently serving, whether you've served before or whether you've never served before.
Part-Time Opportunities
When serving part-time as a Navy Reserve Nurse, your duties are carried out during your scheduled drilling and training periods. During monthly drilling, Nurse Corps Officers in the Navy Reserve typically work at a location close to their homes.
For annual training, you may serve anywhere in the world, including locations in the U.S., at bases overseas, or in areas where humanitarian needs are great.

Most of what you do in the Navy Reserve is considered training. The basic Navy Reserve commitment involves training a minimum of one weekend a month (referred to as drilling) and two weeks a year (referred to as Annual Training) - or the equivalent of that.
Nurses in the Navy Reserve serve in an Officer role. Before receiving the ongoing professional training that comes with this job, initial training requirements must first be met.
For current or former Navy Officers (NAVET): Prior experience satisfies the initial leadership training requirement - so you will not need to go through Officer Training again.
For current or former Officers of military branches other than the Navy (OSVET), as well as for Officer candidates without prior military experience: Officer Candidates are required to attend Officer Development School (ODS) in Newport, RI. ODS is a five-week program that provides a comprehensive and intense introduction to the responsibilities of Navy Staff Corps Officers. Officers who previously held a commission in another United States Military Service, National Oceanic and Atmospheric Administration, Public Health Service, or United States Coast Guard are exempt from attending ODS or LDO/CWO Academy.
With flexible training options, Nurses in the Navy Reserve can comfortably balance civilian and military schedules. Additionally, professional nurses who choose to serve as Reserve Sailors in the Navy Nurse Corps may qualify for special offers. The Clinic Manager provides leadership and oversight of day-to-day clinic operations. Under the direction of the Medical Director and in coordination with the Chief Executive Officer and Chief Financial Officer, the Clinic Manager ensures operational efficiency, financial stewardship, and compliance with HRSA, FTCA, OSHA, HIPAA, Patient-Centered Medical Home (PCMH), and other accreditation and regulatory standards.
This position provides direct supervision to clinical and patient service staff in the medical department, ensuring adherence to organizational policy, HRSA Operational Site Visit (OSV) requirements, and best practices in patient-centered care. The Clinic Manager is instrumental in advancing SDAIHC's mission, vision, and goals, supporting quality care delivery, and ensuring alignment with FQHC standards and Uniform Data System (UDS) reporting requirements.
Essential Duties and Responsibilities:
Manages daily clinic operations including staffing, facilities, supplies, and patient experience.
In conjunction with clinical leadership team, recruits, trains, supervises, and evaluates clinic staff including medical, administrative, and support personnel.
Coordinates staffing schedules and communicates changes with appropriate notice to the Medical Director.
Oversees and coordinates medical personnel onboarding and ongoing compliance with training and competency requirements
Ensures smooth functioning of referrals, screenings, scheduling, and front office processes in alignment with PCMH standards.

In collaboration with the Medical Director and NP Clinical Operations Lead, establishes systems and workflows consistent with SDAIHC policies and HRSA guidelines and monitors for staff adherence to those systems and workflows.
Lead supply maintenance and monitoring, maintaining use of the IssueTrak system, in coordination with the NP Clinical Operations Lead and Medical Director, to assure all broken, lost, or non-performing equipment and supplies are quickly reported and repaired or replaced and Medical Team is aware of status as needed.
Serve as Recall Coordinator in overseeing response to recalls of medication, medical and non-medical supplies, equipment and other items in use in the clinic. Collaborate with Clinic RN to complete related tasks and communications, reordering, etc.
In collaboration with Medical Director and NP Clinical Operations Lead, monitors personnel, facilities, and protocols for effectiveness and continuous improvement.
Develops leadership skills of staff through delegation and empowerment while ensuring accountability.
Participates in quality improvement initiatives, including HRSA UDS reporting, FTCA risk management activities, and PCMH performance metrics.
Provides oversight to Employee Health functions of designated RN staff and identifies quarterly appointments available for staff, assures maintenance of compliance in conjunction with HR
Ensures staff adherence to compliance policies, training completion, and documentation readiness for site visits, including but not limited to HRSA OSV, accreditation visits, and payor site visits.
Protects highly sensitive and confidential information in compliance with HIPAA and SDAIHC policies.
Adheres to and enforces all OSHA, HRSA, FTCA, and SDAIHC safety and compliance policies.
Supports management of medical clinic budgets, revenues, expenses, and payroll as delegated by the Medical Director and in coordination with the CFO.
Minimum Qualifications:
Graduate of accredited school of nursing
Current/non-restricted CARegisteredNurse licensure in good standing in the State of California
Bachelor's degree required (Master's preferred) in healthcare administration, business management, public health, or related field.
Minimum two years proven healthcare management experience in a medical, behavioral health, or community health setting.
Experience in hiring, training, and managing clinical staff and teams including medical assistants, clinic and Case management RNs, and administrative staff.
Demonstrated knowledge of FQHC and PCMH standards, including UDS quality reporting.

Coordinate clinical care for ECM Program clients and establish care plan goals.
Organize and monitor services needed by recipients.
Perform bio-psycho-social assessments and develop care plans.
Oversee care plan implementation and monitor treatment adherence.
Provide coaching for behavior change and health improvement.
Document interactions and interventions accurately.
Collaborate with executives and assist in staff training for safety and quality assurance.
Design and execute training programs for Lead Care Managers.
Prepare formal responses and reports for regulatory agencies.
Establish effective processes and ensure compliance with regulations.
Assist in maintaining operational efficiency and quality care for patients.
Performs other duties as assigned.
Education and Experience:
RegisteredNurse with a valid CA license from an accredited nursing school.
Associate degree in Nursing from an accredited institution required.
Bachelor's degree in nursing from an accredited institution preferred. Experience in a Health Care Organization or experience in Managed Care setting preferred.
1-2 years of case management experience with low-income populations, focusing on complex chronic conditions, mental illness, and homelessness.
2 years of experience with homeless, low-income, and mentally ill populations, including substance abuse.
Experience in Medicare and Medi-Cal environments preferred.
Required Skills/Abilities:
Bilingual in English / Spanish Required
Proficient in evidenced-based communication techniques like Motivational Interviewing.
Understanding and sensitivity to multicultural communities.
Knowledgeable in self-management practices for chronic conditions.
Skilled in phone-based engagement with members and providers.
Proficiency in data interpretation and learning new software systems.
Strong attention to detail, analytical thinking, and interpersonal skills.
Ability to work autonomously and as part of a team.
Exceptional organizational and time management abilities.
Compassionate and empathetic approach to patient care.
Ability to analyze situations and act ethically and in compliance with regulations.
