* Coordinates and oversees all aspects of patients' home care.
Collaborates with patient care teams to develop and implement care plans.
Educates families on patient condition, needs and care.

Qualifications
One year of clinical experience as a registerednurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.*
Current RN license, specific to the state(s) you are assigned to work.
Minimum six months home health experience.
Current CPR certification.
Valid driver's license, reliable transportation and liability insurance.
Able to assess patient status and identify requirements relative to age specific needs.
Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

Initiates and coordinates the plan of care
Documents problems, appropriate goals, interventions, and patient/family response to hospice care
Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily
Instructs and supervises the patient/family in self-care techniques when appropriate
Maintains accurate and relevant clinical notes regarding the patient's condition

Identifies at-risk populations using approved screening tool(s) and follows established care and reporting protocols

All infectious disease screening will be offered to program participants by the Odyssey House Community Health Center (OHCHC). Nurse will coordinate testing, treatment, and ongoing care within both OHCH locations
Ensure compliance with HIPAA Privacy Rules
Participate in weekly one on one meetings and monthly in-services
Work collaboratively with other departments including but not limited to Accounting, Human Resources, Development, and Compliance and Quality Assurance
Participate in clinical discussions
Complete incident reports and ensure nurses are completing Incident Report per agency policy, ensure all incident reports are submitted to Program Manager/Nurse Practitioner Supervisor promptly
Always maintain professionalism with staff members and clients
Coordinate/facilitate patient care, work collaboratively and maintain communication with patient care team (providers, counselors, etc.) to provide effective, timely, and appropriate patient management
Meet with Community Health Center

Duties & Responsibilities
It is the duty and responsibility of the RegisteredNurse to:
Assesses patients' conditions, monitor vital signs, administer medications and treatments, and ensure patient safety
Develops and implement individualized care plans, collaborating with other healthcare professionals to ensure comprehensive and effective patient care
Educates patients and their families about health conditions, treatment options, and preventative care. They also advocate for patients' needs and ensure their voices are heard
Work closely with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure a smooth transition of care
Administers medications, including injections, IVs, and oral medications, ensuring accuracy and safety
Monitor patients' responses to treatment, assess their progress, and adjust care plans as needed
Prepares to respond to emergencies, providing immediate care and support to patients in critical situations
Stay up to date on the latest advancements in healthcare, participate in continuing education, and maintain their professional certifications
Document all pertinent health related information about clients on the patient chart that is done routinely and according to nursing standards of practice
Complete all incident reports before the end of the scheduled shift if applicable; and
Maintain HIPAA guidelines; and
Perform other duties as assigned.
MINIMUM QUALIFICATIONS
CPR certification, first aid training, and experience in specific areas like emergency or critical care nursing helpful
Ability to work collaboratively in a multidisciplinary setting
Proficient in electronic health records
PREFERRED QUALIFICATIONS
Associate's Degree in Nursing, Bachelor of Science in Nursing, Nursing Diploma or equivalent

WORK ENVIRONMENT
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam and Washington, D.C., to Washington state. As a Navy Nurse, you could work at one of the highly acclaimed National Naval Medical Centers in Bethesda, MD, Portsmouth, VA or San Diego, CA. QUALIFICATIONS AND REQUIREMENTS
