Job Description
Collaborates with and plays a lead role with Health Care Center and external network PCPs, the Care Team and other practitioners to ensure members are well supported and managed within the Health Home of the Chronic Care Model.
Collaborates with ASO's, PAC Providers and other Community Services as necessary to ensure appropriate access to service and follow up on the results to such referrals.
Responsible to manage and coordinate care for an assigned population of Level 3, high risk members.
Completes assessments and re-assessments timely for members assigned to the Care Team.
With the input of the member, family and medical team, creates an appropriate and timely individual care plan to assist the member to achieve the established goals.
Works to deliver care management services in an efficient and cost effective manner.
Performs PAC assessments and exception request visits as requested
Communicates the care plan to the Primary Care Physician, member and others in the Health Home.
Revises the Care Plan as needed and at a minimum following hospitalization of the member or following the annual re-assessment.
Meets with patients face to face in the home or at physician appointments to complete assessments and other contacts, as necessary.
With the support of the Care Team, re-engages the member following hospital admissions and emergency room use through reassessment, coordination of transition of care, including PCP appointment and transportation as needed.

Role Summary
As the RN NurseManager, you will oversee the day-to-day clinical operations of the Medical Telemetry unit (3236 beds), collaborating closely with the Director of Nursing and Clinical Nurse Coordinators (CNCs). This role is essential in guiding nursing staff, optimizing care delivery, and fostering a culture of excellence and accountability.
Youll serve as a mentor, resource, and role modelensuring exceptional care, regulatory compliance, and continuous improvement in patient outcomes.
Key Responsibilities
Lead and manage daily operations of the Medical Telemetry unit, including staffing, resource allocation, and workflow coordination
Drive nursing excellence through effective leadership, coaching, and team development
Monitor patient care processes to ensure high-quality, evidence-based nursing practices
Collaborate with interdisciplinary teams to optimize patient outcomes and throughput
Support staff education, competency validation, and professional development
Engage in quality improvement initiatives and performance metric monitoring
Act as a liaison between nursing staff and hospital leadership
Ensure compliance with FloridaNurse Practice Act, hospital policies, and regulatory standards
Participate in the recruitment, hiring, and retention of top nursing talent
Foster a culture of safety, service, and shared accountability
Qualifications
Active Florida RN license or compact license eligible under Floridas requirements
Graduate of an accredited nursing program
BLS and ACLS certifications (or obtained within 30 days of hire)
Minimum 2 years of recent charge nurse or CNC experience in an acute care hospital
At least 1 year of full-time hospital nursing experience
Prior management or administrative supervisory experience preferred
Strong leadership, communication, and critical thinking skills required

Unit Size: 3236 beds
Leadership Support: CNCs assist with unit management; reports to the Director of Nursing
Compensation & Incentives
Pay Range:
$38.52/hr $53.70/hr (based on years of experience)
Sign-On Bonus:
Offered on a case-by-case basis
