RegisteredNurse Care Manager
SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants' needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 "on-call" process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.
Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant's health problems.
Performs the duties of Home Care Coordinator on the IDT as needed / assigned. May perform the duties of other IDT members based on professional licensing, competencies, and experience as needed.
Actively participates in utilization review meetings and quality improvement projects / meetings.
Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated.
Participates in family meetings, staff meetings, in-service and training and orientation programs as required.
Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families.
Practices standard precautions and follows PACE Program Infection Control protocols.
Performs other duties as required or requested.
EXPERIENCE, EDUCATION AND CERTIFICATIONS:
Bachelor of Science in Nursing Degree preferred.
State RN License required
NJ: Licensed by the New Jersey State Board of Nursing.
BLS required (must have within 90 days of employment).
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
Experience in home care, long-term care and / or managed care preferred.
1 year experience providing care as an RN required.
Bilingual with Fluency in Hmong required.
PRE-EMPLOYMENT REQUIREMENTS:
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Qualifications
Graduated from an accredited school of nursing with a diploma, Associate or Bachelor's degree in Nursing.
Passed the NCLEX_RN
Licensed by the state.
Must be CPR certified.
Obtain/maintain ONS Chemotherapy/Immunotherapy Certificate within three months of employment.
Obtain Celgene Certification once orientation period completed.

Job Specific Position Duties: The duties include, but are not limited to the following:
CORE Duties:
Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.
Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.
Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM

Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.
Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
In addition to the duties listed in the basic contract, the following additional or supplemental duties are required for HCWs on this TO. The duties for the HCW are as follows but not limited to:
Maintain adherence to Joint Commission, URAC, Case Management Society of America (CMSA), and other regulatory requirements. Apply medical care criteria (e.g., InterQual).
Provide input on MTF CM resources and make recommendations to the Command as to how those resources can best be utilized.
Work in conjunction with the entire healthcare team and other departments, to identify high-risk and/or high-utilizer populations to include but not limited to those beneficiaries with multiple providers, multiple admissions/readmissions, Emergency Department visits, catastrophic illness, chronic or terminal illness, and multiple medical problems/dual diagnoses.
Provide case management advice and consultation.
Collaborate with other members of the healthcare team, the patient and/family/support system on a regular basis to establish and update the case management plan of care using evidenced-based guidelines (when available and/or applicable).
Identify measurable short-and long-term goals/outcomes of care with matching strategies to achieve optimal wellness and autonomy (self- management). Incorporate the patient's cultural background, values and beliefs, readiness to learn and healthcare needs across the continuum of care into the plan.
Provide the patient/ family with the knowledge and skills necessary for the implementation of the established plan. Facilitate patient and family decision-making activities by keeping them well informed of their rights, responsibilities and options. When indicated, follow patients through hospitalization and follows up in ambulatory and community health care settings.
Actively measure the patient's response to the evidence-based plan of care and provide documentation that the plan and the quality of the services offered to the patient correspond to the identified needs.
Ensure appropriate health care instruction to patient and/or caregivers based on identified learning needs.
Facilitate multidisciplinary discharge planning and other professional staff meetings as indicated for complex patient cases and develop a database and knowledge of local community resources.
Develop and implement mechanisms to evaluate the patient, family and provider satisfaction and use of resources and services in a quality-conscious, cost-effective manner.

Facilitate screening and assist with transfers of Exceptional Family Member Program (EFMP) families and service members going through the Integrated Disability and Evaluation System (IDES).
Plan for professional growth and development as related to the case manager position and maintenance of CM certification. Actively participate in professional organizations including participation in at least one annual national CM conference to be funded by the government to be scheduled at the convenience of the government and the HCW. Ensure all training completion documentation is submitted to the region and HQ, as required.
Participate in video teleconferences (VTCs) and other meetings as required.
Provide safe, quality clinical case management services to a variety of eligible beneficiaries, in accordance with the Department of Defense (DoD), BUMED, Regional, directorate, and departmental instructions, policies and procedures.

Perform all charting on the computer, utilizing various programs.
Integrate case management with utilization management and disease management as needed.

* License: Active, unrestricted RN license
