Responsibilities
* Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
Supervises LPNs and HHAs.
Completes documentation timely and accurately.
Regularly communicate patient progress to the clinical manager and care team.
Plans and provides staff education.
Performs on-call responsibilities and on-call services to patients/families as assigned.
Participates in clinical development and continuing education programs.
Other duties as assigned.
Qualifications
One (1+) year of clinical experience as a RegisteredNurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
Current RN license, specific to the state(s) you are assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation and liability insurance.
Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.

Responsibilities
Ensure the overall coordination of home health services provided to the patient is delivered according to acceptable standards of practice and company procedures. Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit. Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees.
Qualifications
Must be a graduate of an approved school of professional nursing.
Must be licensed in the state in which they currently practice, or in accordance with the board of nursing rules for nurse licensure compact for the state in which they practice.
Must have at least two years of nursing experience.
Must have one year experience in home health or hospice.
Must have demonstrated knowledge and understanding of the federal, state, and local laws and regulatory guidelines that govern a home care operation.
Must have basic demonstrated technology skills, including operation of a mobile device.

Previous experience in management is preferred.

Spanish speaking would be great. - Experience: Must be able to do a vaginal exam (if come from teaching facility-they have less skill since medical students/residents do these). Scalp electrodes, Circulate a C-section, Newborn transitions, Assist anesthesiology, IV starts, Blood draws, APGARS assignments-must know, Postpartum hemorrhage control- SIM lab coming-fall 2023, Epidurals, Amnio infusion. Water birth-not a must have, just for knowledge. Preferred: experience with spinning babies. - Are RNs required to titrate drips: Magnesium, Pitocin, insulin, blood. - Is there a Charge Nurse on each shift?: Yes, free charge. - Are there nurse aids/surgical techs on this unit: OB Techs. - OB/Neonatologist in house 24/7: Yes - RT Equipment: BVM-neonates; T-Piece resuscitator. - Are there Tele/Monitor Techs on this unit: Tele on unit- cardiac monitor tech on adult side of hospital- OB RNs do not need to read tele. Charge and relief charges and some core have ACLS. - EFM System: CPN centricity. - Shifts & Scheduling: 6:45-7:15am/pm. - Weekend Requirements: 3 weekend shifts in a 4 week schedule. Noc: Fri/Sat. Day: Sat/Sun. - Holiday Requirements: Typically 2/3 holidays, includes eve/day of. Will be On-Call-as much as you like, not mandatory. Please note in interview notes traveler agrees to holiday requirements (will be assigned 2 out of 3 holidays within contract, holidays are considered the eve/day of). - Is there a call or standby requirement: Low Census On Call. - Floating Requirements: NICU/Peds/Womens as well as MedSurg or ICU with appropriate patient assignment- downgraded, or RDU. L&D nurses may be assigned antepartum patients. They have to be able to take care of baby for two hours after delivery. If they do not have post partum experience they won't be sent there but they have to be prepared that they will take care of antepartum patients. - Scrub Color: Provided hospital scrubs. - How many shifts of unit/department specific orientation: One NEO, one clinical ed/EPIC, one day floor.
Cancellation Policy Client may cancel hours equivalent to the weekly guaranteed hours for the assignment 36, 40, or 48 for a 13 week assignment. For all 26-week assignments, the cancellation policy will be doubled.

Responsibilities
Ensure the overall coordination of home health services provided to the patient is delivered according to acceptable standards of practice and company procedures. Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit. Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees.
Qualifications
* Must be a graduate of an approved school of professional nursing.
* Must be licensed in the state in which they currently practice, or in accordance with the board of nursing rules for nurse licensure compact for the state in which they practice.

OVERVIEW
Responsible for the supervision of client care in the absence of the Program Administrator. Provide support in new participant admissions and collaborate with family
Follow existing nursing care plan to provide the appropriate level of care to all participants
Responsible for completing and maintaining the medication chart which includes name of medication, strength, dose, frequency, route, side effects and special considerations of each medication ordered on each participant by physicians
Assess, evaluate, and document participant responses to services and treatments in the clinical record and up-to-date charting
Provide health education in the provision of care and management of illnesses; provide appropriate community referrals to participants and caregivers, if necessary
Communicate with the physician, as needed, in regard to participant conditions and statuses
Supervise CNAs, Program Aides, and volunteers, as directed
Provide appropriate clinical feedback to the interdisciplinary team if needed
May drive agency or personal vehicle on company business
May perform other duties as assigned
MINIMUM REQUIREMENTS
Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Executive Director of Human Resources.
Current Arizona RN license
Ability to obtain health care certificates including TB clearance, CPR, First Aid, and Food Handlers Certificate
Proven computer skills utilizing Microsoft Office Suite
Proven strong communication skills - oral and written are clear, concise, and in an organized fashion using appropriate style, grammar, and tone
Proven ability to facilitate an interdisciplinary approach for providing quality service to participants
Ability to stoop, climb, bend, and reach
REGULATORY
