The RN Clinical Manager works under the direction and guidance of the Branch Administrator to provide supervision of all clinical staff working with high medical acuity clients and/or clients receiving extended non‐licensed support services. This leader is responsible to provide staff orientation and ensure competence as a Preceptor and or ensure RN Case Manager/Preceptor resources are in place. Ensures that services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures.
Essential Job Functions:
• Plans, develops, and manages the clinical programs, services, activities, and employees of the agency consistent with company policy and regional management directives.
• Assists with the oversight of the agency's growth related to home care.
• Serves as the clinical expert and assists with the operational and financial management of the agency. • Investigate and take appropriate actions on client/consumer complaints.
• Participate in the recruiting, hiring, and identifying the training needs of clinical staff
• Evaluates programs and services regularly to identify opportunities for improvement.
• Conducts regular client home visits to ensure quality of care and performs home visits as needed.
• Ensures client compliance with federal/state regulations through policy and procedure administration to staff.
• Supervises all clinical staff (RN/LPN/CNA) and perform annual employee evaluations for all clinical staff.
• Responsible for overseeing case management to ensure services that are financially sound. • Manage caseload as needed for client coverage.
Requirements:
• Registered Nurse licensure in the state of practice.
• Obtain and maintain active CPR per agency policy.
Preferred:
• Bachelor's degree preferred
• Medicare Skilled Nursing experience and a basic understanding of OASIS
• 3+ years RN experience in a healthcare setting (home health preferred)

The RN Director of Nursing is responsible for clinical and operational functions of the assisted living residence to include managing and determining appropriate human and financial resources to further the organization's goals.
PRINCIPAL ACCOUNTABILITIES:
1) Ensure quality of care and regulatory compliance by providing over-site and direction to all levels nursing staff.
2) Responsible for implementing the nursing process, including assessment, case management, diagnosis, planning, goal setting, implementation and evaluation, in the delivery of professional and paraprofessional services according to MDH licensing.
3) Meet with the client and family to accurately complete all admission paperwork, explaining the home care plan and client/family responsibilities. Participate in care conferences as necessary.
4) Monitor client's funding sources to assure that prior authorization is obtained and transferred accurately and timely to the Prior Authorization Department and that the appropriate payors are billed. Assist with collections as necessary.
5) Supervise and coordinate maintenance of appropriate, accurate and complete documentation of information on clients and staff under your supervision according to agency policy.
6) Delegate appropriate nursing functions according to State Licensure and agency policies and procedures. Maintain compliance with current county, state and federal regulations affecting home health care.
7) Actively participate in agency-wide activities and its continuous quality improvement program. Responsible for evaluating the home care program's level of excellence and implementing process improvement strategies as indicated.
9) Perform other functions as deemed appropriate by the Executive Director or Regional Manager of Operations.
10) Market the wide range of health care services to potential clients and their families. Assist with/support marketing strategies to achieve financial stability through maximum census. Network with other community providers.
11) Develop and maintain relationships with third parties such as county social workers, hospital discharge planners and others to assist in quality care and referral issues. Network with other community providers.
Fiscal Responsibility
12) Participate in the annual clinical budgeting process and work with the Executive Director in developing new and creative ways
QUALIFICATIONS:
Current licensure as a Registered Nurse in the State of Minnesota. Public Health Nurse Certificate and bachelor's degree in nursing, preferred. Minimum of five years' experience in nursing, preferably in an Assisted Living and/or home health care. Prior supervisory/management experience.

* Communicates medical concerns and coordinates necessary follow-up appointments, and/or referrals with interdisciplinary teams both internally and externally.
* Collects and reviews clients' medical data and ensures that medical records are current and accurate.
* Assesses nursing implications of all data pertaining to the resident and determines the need to communicate with the physician.
* Monitors acute and chronic medical conditions of clients; provides treatment or consultation as needed.
* In emergency situations, gather relevant information and assess client status. Provide appropriate care and/or treatment, direct others as necessary, and collaborate referral to a higher-level of care if appropriate.
Treatment Team Collaboration
* Serves as a resource to staff and other interdisciplinary teams regarding client medical concerns.
* Collaborates with the treatment staff to develop individual client treatment plans.
Implement, monitor, and coordinate medication administration to clients
* Verify, count, and administer medications to residents.
* Educates clients on medication side effects, adverse effects, and drug interactions.
* Monitors clients daily for side effects, adverse effects, and/or drug interactions.
* Ensures client prescriptions are filled and the appropriate amount is maintained through each client's stay within the residential milieu.
* Monitors medication logbooks for completion and accuracy.
Staff Meetings & Required Training
* Completes all required trainings that are department/role specific & facility-wide requirements.
* Maintains and stay up to date with trainings and certifications (i.e., online trainings, internal and external in-person trainings, CPR certification, nursing license, first aid and safety, medication administration, and infection control etc.
* Participates in internal training opportunities with community partners.
Client Confidentiality
* Ensures client information remains confidential and compliant with Nexus policies involving client privacy, informed consent and confidentiality standards for clients and their families.
Requirements
Required Education and Licensure:
* Graduate of an accredited school of nursing.
* Valid Registered Nurse License.
* 1 + year related nursing experience
* First Aid/CPR/Infection Control certification
* Valid driver's license required. Must meet state regulating agency and Home Office driving requirements
Preferred Education and Experience:
* 1+ year related nursing experience in a residential setting.

Duties and Responsibilities:
As a Registered Nurse at GlenOaks, your duties will include but are not limited to:
Resident Care Management: Assess, plan, implement, and evaluate resident care plans to ensure the health and well-being of residents.
Health Monitoring: Monitor residents' physical, emotional, and mental health by conducting assessments and reviewing vital signs, labs, and care progress.
Administer Medications: Safely administer medications as prescribed, document medication administration, and ensure compliance with healthcare regulations.
Supervise Care Teams: Provide direction and oversight to nursing assistants and other healthcare team members to ensure residents receive the highest level of care.
Documentation: Maintain accurate and up-to-date medical records, including care plans, progress notes, assessments, and any changes in residents' health.
Coordinate Care: Collaborate with physicians, therapists, and other healthcare professionals to coordinate and update treatment plans as needed.
Resident and Family Communication: Communicate effectively with residents and their families regarding care plans, progress, and any concerns.
Patient Advocacy: Advocate for the needs and preferences of residents, ensuring that their rights are upheld in all aspects of their care.
Emergency Response: Respond promptly and effectively to medical emergencies, following established procedures and providing necessary care.
Compliance: Ensure that all nursing practices comply with state regulations, facility policies, and safety protocols.
Quality Improvement: Participate in quality improvement initiatives and ensure that best practices are consistently followed.
Education and Certification Requirements:
Registered Nurse (RN): Must hold a valid RN license through the Minnesota Board of Nursing.
CPR and First Aid Certification: Required (or must be obtained within a designated time frame after hire).
Bachelor's Degree: A Bachelor's degree in Nursing (BSN) is preferred, but not required.
Continuing Education (CEUs): Must complete continuing education requirements as mandated by the state of Minnesota for license renewal.
