Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
Works closely with Cath Lab patients ensuring their comfort and making them aware of what is going to take place.
Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.

Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
Works as an advocate for the physical and emotional well-being of the patient.
Perform other duties as assigned.

JOB SUMMARY
Serves as a clinical member of the multi-disciplinary team, providing treatment, rehabilitation, and support for client population.

Additionally, RN may take calls from recently discharged clients requesting bridge prescriptions, follow-up calls on weekends, obtain prior authorizations and coordinate with pharmacies.
RN collaborates with Mobile Crisis and Walk in Center assisting with clients being referred to higher level of care, completing S-BARs.

Qualifications
Position Requirements:
Graduate of an accredited nursing program. BSN preferred.
One-year prior psychiatric experience in mental health nursing preferred.
Licensure and registration by the Tennessee Board of Nurse Examiners or Compact as a registerednurse.

The unit provides medication management and support for situational depression and ideation, while focusing on maintaining the independence of most patients.
Key Responsibilities:
Provides patients with detailed cardiac education and helps patients establish goals for better quality of life.
Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
Documents patient care given.
Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.
Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.

Provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.
