This role is a focused on both home health clinical quality assurance and home health clinical operations initiatives. Develops, plans, implements, analyzes, and organizes clinical operations for a specific location managed. Conducts/delegates the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies. Manages the assignment of caregivers. Responsible for and oversees the delivery of care to all patients served by the location. Receives case referrals. Reviews available patient information related to the case, including disciplines required, to determine home health or hospice needs. Accountable to ensure patients meet admission criteria and make the decision to admit patients to service. Assigns appropriate clinicians to a case, as needed. Instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing patient Plan of Care (POC). Monitors cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers. Ensures final audits/billing are completed timely and in compliance with Medicare regulations. Identify urgent or emergent issues and the appropriate care level (e.g., immediate treatment, appointment scheduling, or referral to a specialist). Establish triage protocols to categorize patient needs, ensuring that more urgent cases receive immediate attention while less urgent cases are scheduled for later appointments. Follows individual provider protocols to assist patients with current issue, requests. Advise patients on how to manage their symptoms at home, explain procedures, or prepare for further testing or appointments. Assist with coordination of patient care with rehab facilities, surgery centers, testing facilities and hospitals. Communicates to clinic patient needs, issues, requests from outside physicians and health care facilities, utilizing the appropriate method of contact such as phone call, Athena text or Case Note to facilitate patient care. Be familiar with requirements for precertification of surgeries, physical therapy, MRIs and other tests. Maintain accurate and comprehensive documentation of the patient's condition, medical history, triage decisions, and any advice or care provided. Monitors patient portal to facilitate requests received are addressed. Provide clear instructions and offer support during the triage process. Ensure that patients understand the next steps in their care plan and provide reassurance as needed. Maintains patient confidentiality and ethical practice. Must comply with HIPAA confidentiality standards when accessing or communicating patient information. Ability to multi-task, effectively managing time and prioritizing tasks. Required Qualifications: A valid RN, LPN, ATC or Medical Assistant license. Previous nursing experience in a clinical, urgent care or ER setting preferred.
