JOB SUMARY: To act as patient advocate, providing continuity of care designed to meet individual patient needs through collaboration with other members of the health care team. What You'll Do as a Hospice RN / RN Case Manager: Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
