This team of Case Managers will provide in-home assessments, care plan development and coordination/ monitoring services to determine proper level of care for functionally and chronically disabled adults and children who are at risk of institutionalization. OMPENSATION & PAY STRUCTURE This position is compensated based on actual billable time worked and may be engaged as either a W-2 hourly employee or a 1099 independent contractor, depending on eligibility and engagement structure. Compensation is provided for approved, billable activities only, such as participant assessments, required documentation, and authorized care coordination tasks. Payment is calculated by the exact amount of time spent completing each billable activity. ESSENTIAL DUTIES: In this role, you'll be a crucial guide and advocate, shaping how participants interact with support systems. Here's what you'll do: Conduct comprehensive assessments: Analyze medical and psychosocial information, conduct home visits, and evaluate the participant needs and quality of services. Develop personalized care plans: Help participants access home and community-based services that empower them to maintain independence in the least restrictive environment. Coordinate and connect: Refer participants to resources, consult with professionals, and coordinate services to ensure a seamless support system. Champion participant autonomy: Advocate for participant rights, promote self-determination, and ensure fair access to vital services. Document for impact: Maintain timely accurate records to track progress, inform service decisions, and contribute to the ongoing improvement of the CPAS program. EDUCATION AND LICENSING REQUIREMENTS:Holds a valid State of Maryland RN license. Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment. Has 2 years of experience in medical-surgical/geriatric and/or pediatric case management. Must be able to pass a thorough background investigation.
