Our client is currently seeking an Adult Care Facility Complaint Investigator (Clinical) to join their team. This is a full-time, long-term contract position.
Responsibilities Include:
Conduct on-site and off-site complaint, incident, death, and questionable operations investigations at facilities as assigned.
Follow all NYSDOH-established protocols pertaining to the assigned investigation, including but not limited to:
Interviewing residents, staff, and witnesses
Conducting records reviews
Site inspection
Investigation closure process.
Document all investigation activities in the appropriate data system(s) following all documentation timeline criteria.
Participate in closures and emergency events as assigned.
Attend and complete all training, both in person and remotely, as required.
Obtain Surveyor Minimum Qualifications Test Certification within one year of employment
Qualifications:
Registered Professional Nurse (RN) licensed and registered in the State of New York with:
One (1) year of survey or investigation experience, or;
Three (3) years of professional nursing experience in a post-licensure professional nursing experience, with at least two (2) years of which must have been professional clinical nursing experience in a licensed healthcare facility. A bachelor's or master's degree in nursing can be substituted for one year of the required general nursing experience.
Licensed Practical Nurse (LPN) licensed and registered in the State of New York with two (2) years of survey or investigation experience.
Surveyor Minimum Qualifications Test Certification, preferred.
Ability to work both independently and as a part of a team.

Manage arterial lines, central lines, and PICC lines.
Administer blood products and medications, including emergency medications.
Provide care for ventilated patients and assist with chest tube insertion and removal.
Interpret arterial blood gas results and dysrhythmias.
Monitor and titrate cardiac and vasoactive drips.
Collaborate with a multidisciplinary team to support patient recovery.
Required Experience / Certifications / Licensure
Active New York Single State RN license (NY is not Compact).
ACLS, BLS, and NIHSS certifications required.
Minimum of 2 years of experience in a CVICU or similar setting preferred.
EPIC charting experience is a must.

Position Overview:
Under the direction of the Nurse Manager, the Level II Registered Nurse is responsible for delivering expert, compassionate care to critically ill patients in the Stepdown/PCU setting. You will play a crucial role in maintaining clinical standards, supervising patient care activities, and contributing to a multidisciplinary team. Your responsibilities may also include serving as charge nurse, preceptor, or committee member, supporting both patient outcomes and team development.
What You Bring:
Proven experience in Stepdown, Progressive Care Units (PCU), or similar high-acuity settings.
Proficiency in neuro checks, including Q1 neuro assessments, and managing neuro-specific equipment such as EVDs, lumbar drains, sub-dural drains, and VP shunts.
Hands-on experience with critical interventions including 3%NS and Cardene drips, arterial line management, carotid endarterectomy procedures, aneurysm coil/clips, Deep Brain Stimulator (DBS) insertion, and seizure management.
Strong telemetry skills and the ability to monitor and interpret complex patient data.
Excellent clinical judgment, attention to detail, and a patient-centered approach.
Preferred Education & Experience:
Education: Associate's or Bachelor's degree in Nursing (BSN preferred).
Experience:
Minimum of 2 years of recent RN experience in a Stepdown, PCU, or Neuro ICU setting.
Experience with high-acuity stroke care and neurosurgical patients.
