We are currently hiring for a Field RN in our Syracuse Area! As a Field RN at Venture Forthe, you will be responsible for the medical oversight of clients in their homes. This position is full-time, with occasional on-call responsibilities.
***Sign on Bonus up to $10,000***
This number is based on the number of hours the individual is working on a part-time basis. For example, if working in a Full-time 40 hours/week capacity, the individual would be awarded $10,000. If the individual works Part-time 20 hours/week, the amount that would be awarded would be $5,000.
RN hires will receive bonus payments, equally divided upon the bonus given:

• After six months of continuously working.
• After 270 days
• After one year of employment
Mileage & Car Stipend if regularly traveling 60+ minutes (one way)
Requirements:
Job Requirements
NYS licensed and currently registered as a Registered Professional Nurse.
Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
Ability to speak effectively before customers/patients or employees of the agency.
Excellent organizational writing, communication and interpersonal skills.
Ability to apply common sense understanding to carry out instructions furnished in written, verbal or diagram form.
Required to participate in on-call rotation.
Must have 1 year Home Care experience
REGISTERED NURSE (RN) DUTIES/RESPONSIBILITIES:
Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts, and maintaining an appropriate Plan of Care $35/hour +$7/hour for on call pay
Communicate with referrers, discharge planners, and physicians to ensure seamless patient care.
Develop, implement, and monitor individualized treatment plans, ensuring proper care coordination and medication management.
Perform in-home patient assessments, reassessments, and provide intermittent nursing services.
Ensure accurate documentation and compliance with agency policies, including patient rights and discharge planning.
Supervise and provide in-service education for staff, conducting competency evaluations.
Participate in quality improvement programs and policy reviews.
Provide 24-hour on-call nursing coverage on a rotating basis.
May assist with PCA Training Program as Needed
