Three (3) years of Home Health experience, Two (2) years of Clinical experience Required
RN Home Health experience preferred. This is a leadership role.
Summary
Responsible for assisting the Director of Clinical Operations and Administrator in planning, organizing and directing of patient care services in accordance with current standards and regulations. This position is responsible for the overall supervision and coordination of an interdisciplinary team of staff to ensure the quality of care is delivered safely to home health patients. The Clinical Manager directs patient assignments, staff scheduling, and coordination of referrals, patient visit volume distributions, and assists in meeting financial targets by effectively managing utilization, productivity, personnel and supplies. This position ensures adherence to clinical best practices to promote optimal patient outcomes and patient experience and ensures that patient needs are continually assessed through development, implementation and updates to the individualized plan of care in coordination/collaboration with the Case Manager, physician and patient.
Requirements
Education:Bachelor's degree required, masters preferred.
Experience:Minimum of 2 years clinical experience and 3 years Home Health experience required. Demonstrated leadership skills or experience required.
Certification:BLS (American Heart Association / Basic Life Support) required.
License:State of Florida Registered Nurse License or Physical Therapist License or Occupational Therapist License or Speech-Language Pathologist License required.
Other:Must establish and maintain positive and effective working relationships with staff, physicians, patients and other departments. Proficient computer skills.

* Ensure that all verbal orders, including telephone orders from attending physicians, are received by a registered nurse. If no RN is available, receive all telephone orders from physicians.

Assist Nursing Program Specialist in ensuring that all assessments are completed prior to the Interdisciplinary Resident Care Plan Committee meeting.
* Check all documentation related to assessment for those residents scheduled for Care Plans.
* Ensure that any incomplete documentation is finished within twenty-four (24) hours of scheduled Care Plan Conference.
Participate in economic utilization of supplies and ensure that equipment is maintained in a clean and safe manner.
* Assist with or institute emergency measures for sudden adverse developments in patients such as cardiac arrest.
* Participate in the maintenance of a clean and safe environment.
Actively participate in continuing educational programs for self-development and in overall quality improvement programs.
Perform other related duties, including special projects, as assigned.

Performs initials assessment and comprehensive OASIS assessments on multi-discipline cases, obtain necessary authorizations and consents and develops and institutes the clients' care plan at admission, resumption of care and recertification visits.

Develop and implement, with physician and patient, an individualized Plan of Care based on patient's physical, medical, psychosocial, and environmental needs, and make appropriate revisions as the patient's condition changes. Supervise the Licensed Practical Nurse (LPN) through instruction on initial Plan of Care as established by the qualified registered nurse. When indicated, the Registered Nurse will make an on-site visit with the LPN/LVN; for certified only.

Observe and report signs, symptoms, and progress of patient to physician.
Coordinate services, informing physician and other interdisciplinary team members of changes in the patient's condition and needs.
