Coordinates, and documents patient care in a knowledgeable, skillful, and consistent manner. This includes patient assessment, education, medication administration, treatments, and patient safety.

Minimum Requirements
Education - Associate degree in Nursing (AD N). BSN preferred.

Essential Functions

Under the guidance of a physician and/or a Certified Wound Care Clinician, is able to perform assessment/ reassessment of the physical and educational needs of patients with any chronic wound needing advanced wound care techniques. Communicates patient needs to appropriate members of the wound care team. Provides discharge teaching as required. Assists with referrals for studies, labs, procedures, and other physicians.
Under the guidance of a physician and/or Certified Wound Care Clinician, formulates, implements and evaluates an individualized plan of care based on patient needs. Through consultation and collaboration with the members of the wound care team, coordinates care in an efficient, cost-effective manner using knowledge and understanding of current advanced wound care modalities and practices.
Assists with education of patients and their caregivers on current evidence based advanced wound care modalities and practices. Assists with promoting knowledge of healthy practices in area of expertise throughout the community through education.
Identifies actual/potential issues of resource management/patient care; works with wound care team to identify cost-effective alternatives; facilitates discharge.
Keeps documentation via EMR current including accurate documentation of products and facility charges.
Maintains safe, comfortable, clean and therapeutic environment for patients and families per established departmental polices.
May perform hyperbaric oxygen therapy related duties, as needed, after completing the required hyperbaric training.
Performs other duties as assigned.

This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred.
Experience - Two (2) years nursing experience. Experience in skin and wound care. Wound Vac experience preferred

As the Registered Nurse in Home Health you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Primary Responsibilities:
Clinical Competence
Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations
Provides required supervisory visits
Documentation and Care Delivery
Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other health care team members
Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely
Documents patient visits per policy and payer requirements, and syncs timely per LHC policy
Quality
Makes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentation
Communicates relevant information timely and effectively with appropriate agency staff, including patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and coordination with other clinicians
Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence
Teamwork
Takes direction from Clinical Director and Executive Director professionally and completes assigned tasks timely, including required learning
Assists in the orientation of new agency personnel and serves as a preceptor to other staff and students
Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes
Participates in on-call and weekend rotation as needed to meet patient needs
Adheres to and participates in the agency's utilization management model

Required Qualifications:
Current and unrestricted RN licensure in state of practice

8 East - Prisma Health Richland 8 East cares for avariety of medical-surgical, orthopedic, and trauma patients. They serve Prisma Health's Total Joint Program, caring for patients with total hip and total knee replacements. They also care for patients with various other orthopedic injuries and spinal surgeries, including those with traumatic orthopedic injuries. 8 East is the designated destination for trauma patients when 7 East is at capacity and cares for a number of patients with various traumatic injuries.
Provides clinical direction for all aspects of patient care in an acute inpatient setting, fostering patient/family centered care. Demonstrates clinical competency utilizing evidence based practices/research in all areas of the nursing process: assessment, planning, implementation, evaluation and outcomes. Consults with clinical advisor and/or peers as appropriate.Adheres to all compliance and policy/procedures of the organization including, but not limited to: licensure renewal, assigned training, employee health screenings, time and attendance policy, dress code policy, patient confidentiality, infection control, medication administration. Demonstrates knowledge of regulatory agency requirements (JCAHO, DHEC, CMS). Provides care within the scope of the South Carolina Nurse Practice Act. The acute inpatient setting usually requires staff to be scheduled for a significant amount of shift, weekend, holiday and on-call work.

Performs the activities within the Prisma Health Patient Flow Operations department, including Central Bed Management and the Transfer Center. Central Bed Management is responsible for the placement of inpatient/observation patients, following the review of admission/discharge/transfer (ADT) orders and bed requests, in conjunction with physician assessment. In addition, the Transfer Center is responsible for the facilitation of direct admits and patient transfers from Prisma Health facilities, as well as external health care facilities and physician practices. Furthermore, Central Bed Management and the Transfer Center coordinate patient admissions, transfers and placement within the capacity and capabilities of each facility/unit.

Standardizes processes to ensure patients are admitted to the appropriate unit with the appropriate orders.
Hardwires standardized processes for inpatient transfers and patient discharges.
Evaluates bed turnaround times and develops action items to address opportunities.
Supports efforts to improve patient throughput and ensure safe, timely care transitions for our patients.
Strengthens utilization of current systems to support patient throughput.
Collaborates with the Emergency Department, inpatient units, Administrators on Duty, and Environmental Services to improve care transitions.
Performs other duties as assigned.

This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Education - Graduate of an accredited school of Nursing program recognized by the SC State Board of Nursing. Bachelor's degree in Nursing preferred
Experience - Three (3) years Acute Care Nursing experience

PACU nursing involves the care of patients who are recovering from surgery, particularly when the application of anesthesia is involved. Once the patient sufficiently recovers from the effects of the anesthesia, he/she is typically transferred to the appropriate ward or discharged from the hospital. PACU nurses are typically responsible for providing patient care during the immediate postoperative recovery period. He/she typically have training in basic life support measures, airway management, acute surgical wound care, catheter procedures and the special requirements associated with anesthesia. PACU nurses may also be trained in the use of cardiac life support equipment.
