BAYADA Home Health Care is seeking a full-time Physical Therapist (PT) to fill the position of Rehabilitation (Rehab) Manager in our Honolulu, HI office. As Rehabilitation Manager you will lead the field staff in providing superior quality client service, clinical excellence, and adherence to policies and procedures. Rehabilitation Managers are also accountable for internal case management, field staff evaluation and development. The Physical Therapist should have prior experience in Medicare Home Health. Rehabilitation Manager (must be a licensed Physical Therapist)

Hours: Full time position

Salary: Annual salary plus bonus opportunities

Responsibilities:

* Direct and coordinate the care provided by a multi-disciplinary care team
* Lead field staff in delivering exceptional client service and quality clinical care
* Ensure adherence to policies and procedures
* Review clinical documentation, including OASIS and care plan coding
* Monitor episodic utilization
* Act as internal case manager and client advocate
* Participate in hiring, interviewing and orientation of field staff
* Participate in on-going education, evaluation and development of field staff
* Ensure accurate and timely case management, including supervision, authorization management and interdisciplinary communication
* Monitor Quality Assurance
* Visit clients to conduct clinical assessments and supervise staff as necessary
* Assist in developing Clinical Associates to become Clinical Managers

Qualifications:

* Current license in good standing as a Physical Therapist (PT)
* Prior supervisory experience
* Minimum of two (2) years community health experience
* Prior clinical and managerial experience within a Medicare Certified Home Health agency
* Comprehensive knowledge of OASIS and the Medicare Conditions of Participation for Home Health
* Strong clinical assessment and case management skills
* Demonstrated success working independently while taking on increased responsibilities

Dignity Health East Valley Rehabilitation Hospital is a joint venture operation and free-standing Post-Acute Inpatient Rehabilitation Hospital

between Dignity Health and Kindred Healthcare. Our two-story, 58,000-square-foot facility includes a stroke unit, therapy gym and 50 beds to

serve patients recovering from significant neurological injury, illness and trauma and similar medical conditions.

The Therapy Manager is responsible for assisting the Director of Therapy Services with the ongoing assessment and improvement of Therapy Services. Such responsibilities encompass not only the internal functioning of the department, but also the integration of the department into the overall functioning of the organization and its clinical programs. The Therapy Manager works in collaboration with the Director of Therapy to ensure optimal functioning of both inpatient and outpatient (if applicable) therapy departments. The Therapy Manager will also assume responsibility and accountability for a designated group of patients and provides therapy evaluations, modalities and treatments, etc. in accordance with established hospital and departmental policies and procedures, maintaining the highest degree of quality patient care.

The Manager participates in performance improvement and program development activities. Responsibilities include providing input into the budgetary planning process; contributing to cost effectiveness of services and programs provided by the department; communicating the employee's observations and suggestions related to opportunities for improvement; and maintaining positive relations with all customers.
ESSENTIAL FUNCTIONS:

♦ Provides an environment conducive to safety for patients, visitors and staff. Assesses the risks for safety and implements appropriate precautions. Complies with appropriate and approved safety standards.



♦ Utilizes the appropriate leadership skills in directing, delegating, and evaluation management Practice. Applies management knowledge principles in order to comply with regulatory Agencies, Federal, State and hospital standards.



♦ Coordinates and integrates inter-departmental and intra-departmental services.



♦ Develops and implements policies and procedures that guide and support the provision of services.



♦ Continuously assesses and improves the quality of (care and/or services) provided.

1. Works with the Administrative Director to plan, develop, and recommend departmental budgets, revenue, capital and operations expenses, sufficient number of staff and any other plans for allocation of fiscal or other resources according to the scope of the various departments.

2. Assist in the development of an appropriate treatment plan and modifies plan according to patient needs.

3.Delegates and/or administers direct patient care, utilizing various techniques as indicated according to performance improvement standards.

4.Through annual Performance Evaluations and daily interaction, reviews the performance and skills of professional staff, making suggestions and corrections when required.

+ Current ACLS

Experience

+ 5 years of experience in the area of discipline plus 3 years of supervisory or lead experience.
