The Senior Therapist is responsible for assisting the Director of Rehab in the efficient Clinical management of rehabilitation services in their assigned account(s). Also, this position assists in integration of rehabilitation services in all settings. The Senior Therapist assesses the need for, develops, and delivers therapy programs to facilitate rehabilitation. The Senior Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of the patients. They implement individualized therapy programs designed to restore, reinforce, and enhance programs. In addition, the Senior Therapist instructs, educates, and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. They direct activities to promote and maintain health. The Senior Therapist supports the delivery of the highest standard and quality of rehabilitation services.
1. Assists therapy staff in effective and efficient delivery of rehab services; adherence to an established plan of care, department and patient(s)' schedule, and timely documentation of services.
2. Assists in clinically managing Powerback Rehabilitation personnel and consultants within facility or home care contracts.
3. Continues direct patient care.
4. Supports the delivery of the highest standard of rehabilitation services through appropriate utilization of resources, the promotion of clinical programs, and staff development in collaboration with licensed therapists, the Clinical Operations Area Director, and Regional Clinical Director.
5. Assists DOR with customer service.
6. Ensures that the clinical management of the rehabilitation department, including oversight related to case management, quality improvement, care planning, clinical utilization, and patient identification is overseen by a licensed therapist.
7. Assists DOR in ensuring practice act compliance.
8. Attends facility meetings and is responsible for information sharing at facility meetings at the DOR's direction
9. Recognized as "go to person" from a clinical standpoint in the gym.
10. Assists in clinical development and growth of therapy staff in their own or other disciplines.
11. Assists in identification of areas of opportunity for clinical growth for the discipline or department in collaboration with the Director of Rehab and Regional Clinical Director.
12. Assists the Director of Rehab developing clinical excellence to support customer service.
13. Assists Director of Rehab with promoting good team work, company culture, and diversity within the rehab gym.
14. Assists in developing, coordination and utilization of student and mentor programs.
15. Assists the Director of Rehab with efficient scheduling of the rehab gym.
16. As necessary or applicable, supervision of Physical Therapist Assistants or Occupational Therapy Assistants, temporary licenses, students and support personnel in accordance with state licensure and professional standards.
17. Attends and contributes to patient care, staffing conferences and other related meetings.
18. Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the patient.
19. Consults and collaborates with other members of the health care team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
20. Adapts program and/or plan of care according to the needs of the individual patient.
21. Conducts training programs and participates in training medical, nursing and other personnel in treatment techniques and objectives, consistent with the patient's course of treatment or those in common to the site of service.
22. Performs other related duties as required. Qualifications: 1. They must have a valid Physical Therapy license in the state(s) of practice, or proof of license and eligibility.
2. They must have a Master's degree in Physical Therapy; or
3. They must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or
4. They must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
5. The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
6. A thorough knowledge of Medicare and third party billing is required.
7. They must be able to practice Physical Therapy per state guidelines.

To facilitate discharge planning in line with goals for length of stays related to difficult to place, high-cost and escalated patients. The Complex Care Social Worker will use a multi-step escalation protocol to identify those patients with barriers that prevent an effective discharge within the goals for length of stay. In conjunction with the interdisciplinary team the Complex Care Social Worker will assume responsibility for the escalated patients in accordance with set standards for discharge planning. The Complex Care Social Worker will also monitor the difficult to place and high-cost patient to identify early intervention opportunities to effect a discharge in a timely and cost-saving manner within the accepted utilization review guidelines.

Mobile Crisis Team Clinician

Works assigned PT shifts on the Mobile Crisis Team, providing assessment and crisis intervention services to persons in the community who are experiencing a psychiatric or behavioral emergency and to significant others at the scene. The mobile crisis staff responds to all calls from members of the community and from the police. Persons in crisis needing MCT services may be in the community or seen at Grassroots. Primary Responsibilities:
Works assigned shift and arrives to work 5-10 minutes prior to shift starting.
Triages requests and responds to a variety of sources up to 40 hours a week as scheduled.
Completes a thorough assessment in partnership with other MCT member, police and collateral others for each new client to determine their needs and the best intervention possible.
Demonstrates ability to work with individuals (adults and adolescents), as well as couples and families.
MCT provides follow-up services to identified clients as well as family and or friends of identified client.
Completes iCarol reports for all calls at the end of each call. All calls are documented before the end of shift.
Provides crisis intervention counseling using a variety of interventions including Emergency Petition, voluntary hospitalization, conflict mediation, access to resources, etc.
Secondary Responsibilities:
Assist MCT Program Manager, and Director of Crisis Services with projects or other tasks, as requested.
Participates in program evaluation by sending surveys to recipients of service as per protocol.
Represent Grassroots at health fairs and other community events, as requested.
Uses counseling skills, empathetic listening and warmth to establish rapport with all callers and clients.
Counselor uses the crisis intervention model with all clients as is appropriate.
Communicates with peers on shift all necessary information relating to operations and client updates.
Actively participates in the various formats of Crisis Team and MCT “meetings” (videos to watch, emails to read, webinars, and or face to face meetings) to ensure that up to date information is being communicated to clients.
Positively contributes to the morale of peers and proactively problem solves client cases as well as systemic concerns, offering suggestions for addressing concerns.
Provides training and orientation to new staff as requested in a helpful and thorough manner.
Keeps workplace including MCT vehicle clean and orderly.
Agency-wide Responsibilities:
Participate in supervision, in-service training and staff meetings.
Maintain good working relationship with co-workers and contribute positively to group morale.
Complete all administrative duties including time sheets, leave requests etc.
Participate in special committees or special projects as needed.

Education and Experience:
Master's degree in social work or other mental health related field, with professional licensure required (LCSW-C, LCPC only).

Two years of relevant experience in mental health service with at least 2 years in crisis intervention services, suicide prevention and intervention.

Familiarity with the DSM so that clients with a medical diagnosis are referred to appropriate medical services.
Will be required to obtain a clearance at the Tier 3 level to satisfy IT Level II designation access and enter information to the Alcohol and Drug Management Information Tracking System (ADMITS), if access is to ADMITS is required to fulfill the requirements of this position.

Youth For Tomorrow is a 501 (c) (3) and a faith based, non-profit organization, founded in 1986 by Hall of Fame Football Coach Joe Gibbs. The organization is large, diverse, experienced and committed to serving at-risk youth.
Position Summary
Youth For Tomorrow is looking for passionate and dedicated individuals to provide Residential services to adolescents at our main campus in Bristow, Virginia. We have both full-time and part-time position available. Full-time positions would require weekend availability. Part-time positions require weekend availability, but with the flexibility of choosing every weekend or every other weekend depending on your availability and preference.
Available Shifts Part Time: Saturdays & Sundays 6:30am-6:30pm or 6:30pm-6:30am.
Role and Responsibilities
Oversee physical care of YFT residents.
Development of acceptable habits and attitudes for YFT residents.
Management of resident behavior.
Assist in meeting the goals and objectives of any required service plan.
Responsible for guidance and supervision of the residents/children to whom assigned.
Responsible for the coordinated supervision of the residents to include but not limited to continued awareness and ability to give an account of each resident location upon request.
Confirm census count received from prior on-duty staff prior to assuming floor responsibilities. Census count must agree prior to shift departure.
Maintain shift coverage until the next shift is securely in position. If House Parents/Coordinators are not on duty, a full report is to be given to incoming staff which may take up to 15 minutes plus past then end of the shift time.
Confirm status and location of assigned residents every 15 minutes and if assigned to work overnight shift, every 30 minutes.
Maintain structure and regulations at all times; 24/7.
Completely and accurately document any required forms, particularly the daily log. This is to be completed prior to the ending shift.
Perform waive testing on residents after home visits, AWOL, and as needed with complete accuracy and compliance.
Attend all training mandated by YFT and continuously pursue training opportunities to meet standards set forth.
All duties and responsibilities will support meeting the structured program of care outlined in DBHDS and DSS state standards (DBHDS: 12VAC 35-46-800; DSS: 22 VAC 40-151-720).
Other duties as assigned by the CEO, Director - Program Services, Director - Residential, or Supervisor
Required Trainings: First Aid/ CPR; Handle With Care: Standard Precautions; Emergency Preparedness; Shaken Baby; Mandatory Reporting; and 25 hours of job specific related training.
Qualifications
Knowledge and understanding of YFT mission and values;
Possess the characteristics and experience to collaborate with and provide appropriate care to residents, gain their respect, guide their development, and participate in their overall treatment program;
Ability to support constructive resident-family visitation and resident involvement in community activities;
Ability to provide services in a culturally and linguistically competent manner;
Possess the temperament to work with and care for children, and youth, some with special needs in multiple age ranges;
Ability to accurately collect, review, and analyze waive testing of residents after home visits, AWOL, and as needed
Ability to work effectively with the treatment team and other internal and external stakeholders;
Adequate and continuous ongoing training and supervision
Age 21 or above as per state regulations and standards.
Ability to maintain a stable environment.
Ability to maintain safety, security, and structure at all times.
Ability to maintain the integrity of the program structure in the absence of House Parent/House Coordinator leadership.
Ability to attend, understand, pass, and demonstrate medication management protocols
Ability to obtain and maintain a valid driver's license which meets or exceeds YFT's insurance guidelines.

JOB SUMMARY:
Under the Program Director's clinical supervision, the Rehabilitation Specialist is responsible for monitoring client progress, scheduling medical appointments, and providing clinical support services. The Rehabilitation Specialist is also responsible for participating in assessing individuals for service needs and the development and implementation of the individual's rehabilitation service plan.
RESPONSIBILITIES:
Assists in the development of effective Individualized Residential Rehabilitation Plans for persons with chronic mental illness in compliance with state and agency policies and procedures under the direction of the Rehabilitation Director.
Serves as a liaison and consultant to residential providers, mental health professionals, medical staff, community agency workers, family, care providers, and others involved in program planning, conflict resolution, and on-going clinical support, as directed.
Evaluates client performances and makes recommendations for referrals.
Participates in service delivery to promote client progress. Coordinates services with other community agencies and participates as required in joint planning, assessment, and service delivery.
Monitors overall client progress and prepares reports as directed. Communicates progress summary to appropriate staff and with client consent, other persons involved in client's treatment program.
Provides crisis intervention services as necessary.
Works with advisory groups and human service planning agencies to develop and improve service and integration opportunities for persons with chronic mental illness.
Establishes and monitors client records as required by regulating agencies.
Participates in the fee determination and billing processes and in the collection of fees.
Conducts and attends in-house, program, staff, and training meetings as required.
As directed, serves as a liaison to clients temporarily being served by inpatient facilities or detention centers and when appropriate, participates in the development of after care plans.
Implements VOAC Policies and Procedures and assures program compliance with state regulations.
Participates in the development of discharge plans and provides follow-along services as required.

CPRP Certified psychiatric rehabilitation professional.
The position requires the ability to provide individualized services to persons who are mentally ill. Experience with treatment planning and writing, client assessment, rehabilitative services, the monitoring of service delivery, and clinical documentation is preferred.
Driving and/or transporting clients is a responsibility of the position.
