POSITION SUMMARY Under supervision of the Program Director, oversees the coordination of care, rehabilitation planning and clinical documentation of all deliverable services for all PRP (Psychiatric Rehabilitation Program) consumers. Rehab specialists will provide assessments, clinical documentation, facilitate sessions aimed to enhance and develop consumers in living, learning, working and socializing. This position is accountable for assisting clients in the development and maintenance of healthy lifestyles and management skills. This includes creation of goal-based treatment plans, assisting in crisis management, maintaining clinical documents and active updates on consumers. This position requires a thorough understanding of the clinic's philosophy of treatment, admission process and criteria. This position will supervise program staff as well as working directly with consumers receiving PRP services. The individual is responsible for ensuring that clinical and administrative services are provided for patients and their families.
