Clinical Rehabilitation Therapist
Screen and assess clients to identify and eliminate barriers to self-sufficiency as well as identify and clinically address behavioral health issues. Provide a range of assessment services: provide specialized mental health or alcohol and other drug services including mental health assessment, chemical dependency or dual diagnosis assessment, psycho-social assessment, pre-employment screening assessment, community support treatment or case management. Provide consultation to outside agencies on client problems, assessments and recommendations based on interview and assessment data. Provide consultation to clients related to their specific individual service plan (ISP) defined needs, priorities established or related to recommendations based on clinical assessment of client employability.
Responsibilities:
Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision and values of Catholic Charities, Diocese of Cleveland.
Within scope of practice, implement site/corporation approved treatment practices, procedures or interventions. Conduct a Diagnostic, Needs or Employability Assessment on referred clients. As applicable, monitor psychiatric symptoms of clients. Administer screening tools with client to access vocational and psycho-social needs (i.e., gather information regarding client's disability, functional limitations, work history, social history, educational background, treatment background and special interest) and prepare detailed report of assessment.
Make recommendations as to client's employability and need for additional treatment or support services.
Develop, maintain and monitor assigned clients' ISP: review assigned clients' progress in treatment and communicate with other qualified mental health, AOD and other providers, childcare, administrative, educational staff and referring agency staff regarding the progress of assigned clients. Provide referrals to other services as required or necessary. Monitor client's progress via review of progress reports, meetings and telephone contacts and identifies service plan amendments or need for additional services.
Coordinate services which may include medical, mental health or drug treatment, job training and utilize community resources when available to assist client in treatment engagement, gaining self-sufficiency, meeting goals.
Under clinical supervision, assist and support clients served in crisis situations.
Assist clients in strengthening communication skills: access appropriate physical and mental health care and chemical dependency resources as needed: access additional needs and provide opportunities to build social skills. Consult with other professionals regarding client needs/concerns.
Counsel clients toward increased functioning and develop goals. Advise clients on personal problems, career planning and emotional effects of disability and mutually agreed upon goals.
Provide psychoeducation to clients and family members.
Participate in Quality Improvement activities and service evaluation.
Attend and participate in meetings, supervision and in-service trainings regarding specific areas of responsibility.
Maintain case records according to funder policies and Federal and State regulations.
Maintain professional licensure requirements of the organization, the Ohio Counselor, Social Worker and Marriage and Family Therapist board and/or the Ohio Chemical Dependency Professionals Board.
Adhere to ethical standards specified by the Ohio Administrative Code, Professional Licensing Organization and Catholic Charities Code of Ethics.
Perform other duties as necessary to accomplish program objectives.

Requirements:
Combination of education and experience normally represented by a bachelor's degree and a LSW or LPC.

Qualifications:
Must be a Maryland licensed mental health professional or have CPRP.

Be comfortable meeting clients in their home and community.

Essential Duties and Responsibilities:
The Internship Teacher Advisor, under the supportive supervision of APS's Work-Based Learning Coordinator and Business Community Liaison, will collaborate with APS's Director of Career Connected Learning/Career and Technical Education and Career Connected Learning staff and work directly with key stakeholders including Interns, teachers, and tutees regarding social and emotional needs of students. Intern Support
Monitor and maintain proactive, problem-solving communication with Interns and Career Connected Learning staff to ensure successful internship participation and completion.
Support Intern orientations, training, etc., and offer ongoing feedback for improvement.
Monitor interns' progress and learning milestones during Intern sessions (e.g. resolving iSolved punching in/out and monitoring weekly hours, 1:1 coaching/mentorship, site visits)
Professional Development
Attend required professional development and training sessions (e.g., Intern onboarding, training, and orientation). Attend the two semesters' CTE Teacher PD days each school year.
Reporting

Assist individual in engaging in programming designed to increase daily living skills and personal and social adjustment.

Collaborate with co-workers to design and execute individual's person-centered plans.
Develop vocational training curriculum and/or adult day activities, depending on individuals' program of service, areas of interest, and person-centered plan.
Develop, implement, and document community engagement and vocational activities (i.e. outings in the community) weekly and/or bi-weekly dependent on the ISP and person-centered plan for individuals supervised.
Must drive an agency vehicle to take individuals on outings in the community.
Ensure and advocate for individual rights of the participants on a daily basis.
Recognize and respect individual differences/preferences.
Using electronic record keeping system to write and develop detailed, accurate, and SMART goals and case notes on activities for each individual assigned to your supervision. Electronic records must follow Medicaid and CARF requirements.
Document Unusual Incidents/Major Unusual Incidents (UI/MUI) in case management system. Notification to Rehabilitation Supervisor, family, Support Administrator, and others identified required within compliance requirements.
Maintain a caseload of up to 12 individuals including case notes, ISP revision dates, documentation, recommendations for transition to next opportunities etc. and keep supervision up-to-date regarding individual progress. Caseloads are built based on acuity, agency needs, and individual's skills and interests.
Communicate regularly with individual's Support Administrator through phone calls, email and meetings; keeping the Support Administrator apprised of the individual's activities and progress in the program. Ensure all communications are documented in the case management system.
Communicate effectively to Rehabilitation Supervisor and Support Administrator when span dates are approaching expiration. This should be communicated approximately 30 days prior to expiration to avoid no payments for providing services.
Act as liaison to referring Support Administrator, family, guardians, and other individuals who are actively involved in the individual's life.
Write monthly reports regarding an individual's progress.
Participate in annual and/or quarterly ISP meetings for all individuals supervised.
Participate in department Program Evaluation activities.
Attend and participate in Department, Program, and agency meetings.
Recommend and implement program improvements.
Maintain documentation and case files in accordance with Medicaid and CARF accreditation.
Continue professional development (membership in organizations like ORA or NJSST, workshops, and seminars). Minimum requirement of 15 hours of professional development per year outside of in-service dates. Requirements can be met through RELIAS training modules as well.
Maintain regular attendance according to VGS Policy.
Maintain a safe work environment and follow agency safety policies.
Ensure that individuals are paid under contract and license of federal, state, or county agency or by piecework according to contract, and enters required documentation related to payroll.
Ensure that the participant is kept clean and unsoiled, and in the case of an accident, assist the individual in clean up and changing of their clothes if necessary.
Supervise lunch and break activities, aid in using the restroom, feeding, and other activities for all those individuals supervised or requested by supervisor.
Maintain department area in accordance with all safety task force requirements.
Monitor transportation for all individuals enrolled in the program.
Must be willing to travel to other VGS locations and/or departments to assist in providing services to individuals as needed; specifically community group and voc hab sites, and other career and activities centers as requested by supervisor.

Qualifications
GED or high school diploma is required; associate degree and 2 years of experience or an equivalent combination of the two is preferable.
Previous experience with the DD population is preferred
