Overview of Responsibilities
Performs periodic tours of all hospital buildings and grounds. Visually checks for safety and fire hazards. Makes written reports each shift on findings and irregularities.
Notifies Chief of Police or other police supervisor in the event of an emergency or criminal act that results in an arrest.
Enforces hospital regulations and policies designed to prevent theft and unauthorized access.
Conducts safety checks and tests as required by State and Federal laws for a hospital.
Assists in all emergencies inside and outside of Reid Health and Reid's satellite offices.
Investigates and files reports concerning irregular activities. Promptly reports incidents involving criminal activities.
Patrols parking areas periodically, leaving warning notices on automobiles in violation of parking regulations. Checks fire and emergency lanes to insure they are clear at all times.
Supports the Reid Health Police Department objectives for creating a safe environment.
Assists with patient restraint when necessary.
Directs visitors and answers questions relative to location of services.
Checks all odor reports and locates origin.
Responds to fire emergency calls, assists police, fire, and emergency management teams
Provides escort service, car door unlocks, vehicle jump starts and automobile tire inflation to assist staff and guests.
Supports hospital departmental policies, procedures and missions statements.
Participates in employee health infection control requirements.
Attends staff meetings and in-services.
Responds to Security Codes I & II.
Successfully completes the required 8 week Tier II law enforcement training and all required mandatory training.
Successfully completes all required hospital training
Education/Experience
Education Required: High School Diploma
Education Preferred: Criminal Justice or other relevant degree that would be beneficial to this position. Must be the age of 21.
Certifications: After hire, must successfully complete the 8 week Tier II police academy at the Indiana Law Enforcement Academy unless previous academy completion was made.
Experience Required: Previous security experience. Have or be able to obtain a valid handgun license for the State in which you reside. Should have experience on a Law Enforcement, military police or security

Overview of Responsibilities
Visually checks for security safety and fire hazards.
Make written shift reports on incidents/events and any use of force events
Notifies Inpatient Psychiatry Charge person of any incidents/events or use of force events
Conducts safety checks and tests as required by State and Federal laws for a hospital and behavioral units.
Assists inpatient psychiatric staff with volatile and aggressive patients/family members to ensure a safe environment.
Promptly reports to Reid Health Police Officers incidents involving criminal activities.
Performs contraband checks with inpatient psychiatry staff.
Supports Inpatient Psychiatric objectives.
Assists with patient restraint when necessary.
Will be part of the inpatient matrix mix on the adult or geriatric unit and will be expected to remain on the units for duration of shift as assigned by the Shift Charge Nurse.
Checks all odor reports and locates origin.
Responds to fire emergency calls, assists police, fire, and emergency management teams
Performs role as a trainer for Security Code Classes and MOAB trainer for Reid Health.
Supports hospital departmental policies, procedures, and missions' statements.
Participates in employee health infection control requirements.

Responds to Security Codes I & II located on the 6

floor and other units as permitted.
Successfully completes all required hospital training.
Will assist Inpatient staff in high acuity situations, documentation, and daily living skills as needed to prevent aggressive behaviors or violence.

Overview of Responsibilities
The following list describes the essential duties of this role. Individuals in this role may not perform all these duties, or may perform additional, related duties not listed here.
Identify reportable and non-reportable neoplasms to be included in the registry as outlined by CoC standards and reporting requirements.
Create suspense files for identified reportable neoplasms by abstracting information from the patient medical record to include demographic characteristics, history of neoplasm, primary site, histology, laterality, case finding source, location of diagnosis, managing physician, type of diagnostic confirmation and name of referring facility.
Perform oncology coding and abstraction of identified non-reportable neoplasms for inclusion in the registry database in compliance with standards set by the CoC, Cancer Committee guidelines and departmental standards.
Transfer AJCC staging from EHR into cancer registry database for analytic cases abstracted.
Collect survivorship data for all applicable newly diagnosed cancer patients to include medical status, first recurrence, death and autopsy information from patient medical records or letters to provide end results information.
Qualitative analysis follow ups - Perform a comprehensive review of data entered for each patient to ensure the completion and accuracy of the data elements.
Assist with room and audiovisual equipment set up and coordinating bi-monthly oncology conferences and quarterly CoC meetings.
Maintain competency and stay abreast of current literature and trends by participating in at least one cancer-related educational activity at the local level in accordance with CoC standards.
Perform other job-related tasks as assigned by Cancer Registry Manager.
Education/Experience
Successful completion of Reid Hospital orientation and competency-based skills appropriate through ongoing assessment of competence and educational activities. Written evaluations are done on an annual basis.
CTR-eligible college graduate preferred.
Ability to sit for and successfully pass CTR certification within 3 months of completion of education.

Overview of Responsibilities
1. Leads and/or assists in large groups, small groups 1:1's and individual activities (exercise, arts and crafts, games, current events, sensory, etc.).
2. Assists Recreational Coordinator in assessing and reassessing participants' functional status in relationship to participation in recreational activities.

Overview of Responsibilities
Meets with each patient to assess the patient's needs and establish rapport.
Develops individualized treatment plan for each patient related to activity therapy needs.
Participates in multi-disciplinary team meetings to develop comprehensive treatment plan.
Plans and implements patient activity and education groups designed to meet patient's individual needs and to enhance patient's general quality of life.

Patient/Subject visits - vitals, blood draws, specimen handling & shipping. Must be trained on phlebotomy and IATA certified (or willing to obtain IATA certification within 30 days of hire)
Assist Study Coordinators with subject screening
File study related documents such as the IB, Protocol, etc. Assist with transmitting de-identified patient information or email correspondence between the coordinator and the sponsor.
Maintain Regulatory binders.
Maintain CAP/CLIA/Lab Norms
Breakdown studies for long term storage/move study records to storage as required.
Safety letters- Print, track, and file if they are not done electronically. The coordinator should obtain PI signature for them in case of questions.
Update the ICF log after updated ICFs approval.
Check binders prior to monitor visits to ensure all regulatory documents are up to date, the correct Protocol, ICF, IB and training documents are present, and all historical documents are filed appropriately.
Assist with training for studies as needed.

Fill in personnel on initial DOA log (this will be the same personnel from the submission)
Set up Regulatory binders
Print off training materials if training is not electronic and ensure a sign off log is present. The RA will not obtain the signatures.

Overview of Responsibilities
Transport patients to, from, and within the department as needed. This may include transferring patients to and from wheelchairs, beds, whirlpools, etc.
Assist with patient treatments as directed by therapists or assistants.
Prepare patients, equipment, and treatment areas as directed. May include setting up equipment or patient without initiating treatment.
Monitor patients during treatment, assisting as needed. Summon appropriate person if signs of patient need or equipment malfunctions are present.
Turn off and puts away equipment at end of treatment. Notify therapist or assistant that treatment has been completed.
Fill portable oxygen containers as needed.
Disinfect mats, toys, and other equipment and facilities between and after patient treatments.
Observe patients awaiting therapy or transportation to note any extended waiting time, signs of illness, patient needs, etc. and handle situations appropriately.
Retrieve used Rehabilitation Services equipment from nursing units (Soiled Utility areas) and disinfect.
Clean items in the department Soiled Utility area. Return soiled utensils to Material Processing at least weekly.
Maintain knee CPM units with pads ready for immediate use.
Perform clerical assignments such as copying, making telephone calls or completing documentation as requested by therapists or departmental management.
Prepare whirlpools and assist with hydrotherapy treatments as requested. Will be solely responsible for whirlpool preparation and disinfection on assigned weekends.
Maintain cleanliness of work conditioning, aerobic, and modalities treatment areas.
Maintain adequate linen stock in assigned areas. Notify Linen Services of linen needs.
Remove soiled linen when bags are 2/3 full.
Maintain treatment tables with clean sheets, pillowcases, gowns.
Launder gait belts and hydrocollator pack covers monthly or as needed.
Prepare and disinfect whirlpool tanks and pulsavac's.
Read and adjust temperatures of hydrocollator and refrigerators, splint pan, and paraffin bath daily and record on appropriate logs. Check status of AED daily and record appropriately.
Maintain clinical supply inventory. Process purchase requisitions and packing slips, check deliveries, and store and retrieve items.
Responsible for cleaning, organizing, and stocking supplies in and straightening assigned areas. This involves disinfecting equipment and toys, cleaning mats and refrigerators, stocking linen and removing soiled linen, and keeping the areas organized.
Submit work orders for appropriate repairs.
Perform miscellaneous errands as needed or directed.
Perform minor equipment repair and maintenance as needed. Submit work orders for appropriate repairs.
Assist with telephone coverage and other duties as assigned in front office when requested.
Greet, direct, and assist patients and visitors in and to the department.
Assist with filing and other clerical tasks as directed.
Upon request, adjust hours and/or responsibilities in the absence of other office or aide staff.
This position will be mainly in OP Pediatrics, but there will be coverage in OP adults as needed. In addition, this position does include weekend/holiday rotation on IP/ARU.
