The DESE-Relief staff assists the individual in normative home-based activities and during community-based activities to increase the individual's development of independent living and community living skills. Locations available in Fitchburg and Worcester areas.

Essential Functions:

· Assists the individual in participating in home-based activities and community-based activities, both generic and specialized.

· Works in the home with the individual to help increase daily living skills or independent living skills, including hygiene tasks, chores, meal preparation, money management, etc.

· Works in the community with individuals to help increase community living skills development, which may include learning to use the library, making a purchase at a store, or participating in a group-based community setting like a swim program or the gym.

· Actively advocates for individuals served and demonstrates agency mission and values in all interpersonal and professional relations.

Responsibilities:

+ Provide side by side interventions to Persons that focus on problem solving, skills training, modeling behavior, optimism and encouragement. Assist the team and Persons in providing on-going assessment of symptoms and responses to staff interventions

+ Participate in the development of treatment plans and ensure that the Person is at the center of the process and that the interventions are clinically based. Support Persons in their desire for recovery and rehabilitation in the development of their goals.

+ Support the pursuit of services that are culturally competent, strengths based, recovery based, focused on rehabilitation and are inclusive of Person preferences. Ensure that interventions embrace Person preferences, are evidence based and utilize cultural/community resources.
