Title: CSA-Community Support Associate - Solutions Program

Credentials Required: High school diploma/GED
Special skills/Knowledge required: one (1) year of full-time experience working with individuals who receive services for treatment of a mental health disorder or co-occurring disorder; successfully complete department led or approved training program within six (6) months of hire.
Primary functions:
Conduct activities necessary to live with maximum independence in community-integrated housing.
Ensure successful community living through the utilization of skill training, cueing, or supervision as identified in recipient treatment plan.
Include: reminding recipient to take meds, teach community resource access and utilization, teach emotional regulating skills, ensures coping skills, and interpersonal skills.
Reminding a recipient to take medications and monitoring symptoms and side effects of medications
Meet maximum requirements for comprehensive community support services established in 908 KAR 2.250.
Shifts/Work hours: Monday-Friday, 8 a.m. through 4:30 p.m.
Funding for this position: Funded in part or in whole from Federal, CHFS, and/or other State funds.

Client Overview:

Prefers a familiar, cluttered environment - caregiver must be respectful of his space
Receives ongoing OT, PT, and home health visits
Occasionally needs transportation assistance for errands, groceries, banking, and appointments
Light housekeeping is welcome (dusting, sweeping), but personal items should not be moved

Qualifications
