Responsibilities
As a Home Health Scheduler, You will:
* Greet visitors in a courteous, professional, and timely manner
* Assist with answering incoming calls from patients, staff, physician offices, referral sources, etc., and transfer callers to the appropriate person or department
* Communicate patient information to appropriate staff as needed to provide the highest level of care
* Select an appropriate clinician for patient assignment based upon patient needs, location of the patient, and skill of the clinician
* Provide administrative support for the agency by performing general office duties, including maintaining an adequate stock of supplies needed for staff and patient care
* Support the clinical manager with managing patient schedules to ensure visits are performed in accordance with ordered frequencies and regulatory requirements
Qualifications
* High school graduate or GED
* Minimum of two-year of administrative experience, preferably in home health
* Proficient in the use of Microsoft Office (Word, Excel, Outlook, Internet Explorer) with the ability to learn industry specific software applications

Responsibilities
* Supervises the provision of assistance to person(s) served in order to promote their physical, social, and psychological well-being, including that staff interactions with person(s) served promotes the rights of the person(s) served to achieve an enhanced quality of life.
* Ensures that each person served has an activity schedule developed and implemented and monitors and ensures staff follows procedures to promote optimum health care and behavior supports, including the implementation of services outlined in person(s) served service plan, and staff training.
* Monitors and ensures staff completes all documentation as required to ensure quality services.
* Monitors labor hours and ensures assigned service sites are operating within budgetary guidelines.
* Responsible for client funds, receipts and proper documentation.
* Ensures that staff are deployed appropriately, and that staffing ratios are in compliance with regulatory and service plan requirements.
* Ensures schedules are prepared and posted in a timely manner, and efficiently utilizes staff.
* Conducts timely performance reviews and conducts staff counseling and corrective actions including work Improvement plans and follow-up.
* Provides timely and appropriate feedback and oversight on staff documentation, including daily progress notes, data sheets, and incident reports.
* Assists with the review and tracking of incident and accident reports and participates as needed with investigations involving persons served and employees at assigned service sites.
* Ensures applicable Occupational Safety Health Administration compliance standards are met and all related training occurs. Documents training and compliance in accordance with company and regulatory requirements.

Qualifications
* BA/BS in Business, Health Care Administration or a Social Science degree or High school diploma/GED and equivalent work experience of 3 years
* One year of supervisory experience required.
* Previous experience providing services and supports to individuals with developmental disabilities and/or related disorders preferred.
* Experience in managing systems, processes, and people.
* Must meet all agency requirements for pre-employment as required by company and/or State regulations.
* Based on geographical location, you may be required to be certified as a Food Service Director

Responsibilities
* Oversee daily operations of sponsored residential services within the assigned region, ensuring high-quality, person-centered care.
* Supervise and support regional program staff, including hiring, onboarding, training, evaluations, coaching, and professional development.
* Routinely monitor service delivery through on-site observations, documentation audits, and staff performance reviews to ensure program integrity and compliance.
* Ensure adherence to safety, health, licensing, and regulatory standards in all program environments.

Qualifications
* Bachelor's degree in human services, social work, psychology, or a related field required.
* A minimum of two (2) years of professional experience supporting individuals with intellectual and/or developmental disabilities (IDD), or in a related human services discipline.
* Preferred: Experience in a supervisory, leadership, or program management role within the human services or disability services field.
* Valid driver's license
* Ability to communicate verbally and in writing.
* Must have good organizational skills and attention to detail, proven supervisory skills, and be capable of working flexible hours
* Must be able to travel between service sites, staying overnight as necessary.

Responsibilities
* Provides clinical supervision and training of staff and monitors/implements/supervises delivery of service plans and personal futures plan
* Provides clinical on-call to respond to emergencies

* a graduate of a college or university with a master's degree in a human service field and has one year of full-time, post-graduate degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional who has one year of full-time, post- graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or

Our Company ResCare Community Living A Host Home provider is an independent contractor who opens their home and heart to an adult with a development disability. Depending on the individual, supports will likely include support with personal care and hygiene, meal planning and cooking, medications, recreation, and leisure activities in and out of the home, transportation, shopping and more.
Responsibilities
* Receive the Individual into the Provider's home and to provide the Individual with appropriate daily care, activity, and full-time supervision in accordance with applicable laws, the Individual's Service Plan and with due regard for the Individual's emotional, intellectual, physical, and spiritual needs.
* Prevent any acts of disrespect, abuse, neglect, or exploitation of the Individual receiving services from the Provider and to immediately report any such acts to the company and the applicable state/local agencies.
* Document the Individual's progress and unusual events or occurrences. To assist the company in meeting the company's obligations under state and federal law and regulation, Provider shall complete and submit reports to the company.
* Arrange and establish routine or emergency medical and dental care for the Individual consistent with the Individual's Service Plan and with due regard for the Individual's physical and emotional needs.
* Provide transportation for the Individual to service appointments and /or community activities, consistent with the Individual's Service Plan.
* In the event of serious illness, serious injury, or death, to immediately notify the company's Clinical Supervisor or appropriate on-call staff as soon as possible and after emergency medical or dental care is obtained for the Individual.
* Communicate fully, promptly, and regularly with the company about matters affecting the care or placement of the Individual, as provided by state law and regulations and the Independent Contractor Agreement.
* Maintain all information and records (including logs, notes, etc.) regarding an Individual in a confidential and privileged manner at all times.
* Complete daily notes/grids-thoroughly
* Attend ISP, IEP, or BSP meetings monthly or as scheduled
* Be on time to all meetings, appointments
* Call with 24 hours-notice if cancelling or rescheduling meetings/visits

* Sign off on back of each page of MAR (any provider/staff who administer meds)
* Sign off on MARS daily
* Sign off on PRN's, document results on back of MAR (per each occasion)
* Fill out specific required forms for annual dental, vision, medical checks and turn into QP
* Get orders for Adaptive Equipment from necessary physicians (i.e., glasses, hearing aids, breathing machines/treatments, shower/grab bars, walkers/wheelchairs, etc.) and specific diagnoses.
* Have Physician orders signed by the Primary Physician (yearly with annual)
* Complete labs as ordered by physician or as requested
* Have Physician Orders signed by Psychiatric Physician (upon each visit)
* Any Medical changes need to be updated on the Physician Orders including (Medication changes, Adaptive Equipment, diets, allergies etc.)
* When labs are completed; follow-up and obtain results from Physician (Psychiatric or Primary)
* Complete annual visits and follow up requirements (get clarification from doctor if not sure)-Physical, Hearing (Not required unless hearing issues/concerns), Dental, Vision
* Limited travel
* All other duties as assigned
