Cultivates and maintains on-going strategic client relationships and partnerships within the organization to achieve the revenue, growth, and retention goals of PharMerica/Pharmacy Alternatives. Ensures retention of assigned book of business and drives resident adoption. This position is 75% travel within the territory: Mid-West USA area Applicants can reside within, or a commutable distance to the territory. Schedule: Monday - Friday, hours depend on the business needs The ideal candidate will have 3 or more years' experience in client management/retention or sales in Long-Term Care, facilities with Individuals with Developmental Disabilities (IDD), or related industries We offer: Competitive pay Health, dental, vision and life insurance benefits Company paid STD and LTD Employee Discount Program 401k Paid-time off Tuition reimbursement Non-retail/Closed-door environment Responsibilities * Works with the Director, Account Management in developing strategies that result in retention of assigned PharMerica/Pharmacy Alternative Clients * Works with the IDD team to manage PharMerica's/Pharmacy Alternatives' clients and customers. * Conducts and documents meetings with assigned clients * Works directly with key PharMerica/Pharmacy Alternatives teams to deliver on client commitments and to meet service-level agreements. * Owns client issue escalation and resolution process. * Prepares for and participates in a client review process to include maintaining documents for such review. * Documents Client retention, upselling activities, and interactions, in PharMerica's/Pharmacy Alternatives' designated CRM platform * Owns proving insight and recommendations on pricing negotiation and contract renewals. * Works with other disciplines (billing, credit/collections, contracting, and marketing) to assure that corporate billing, and receivable standards are achieved. * Prepares quarterly business review materials and client engagement call materials as assigned. * Conducts routine client-facing training for PMC/PAL products, tools and resources. * Provides educational materials to facilitate compliance with PMC procedures, tools and resources. * Supports solution selling or up-selling of PMC/PAL products and services. * Ensures an overall positive experience for the assigned client base. * Works collaboratively with clients and key PharMerica/Pharmacy Alternatives management to implement various corporate initiatives, implement PharMerica products/services/tools, and oversee new facility start-ups/transitions. * Compiles Client reports and assists with CRM data management and issue resolution management. * Attend PharMerica corporate/networking events and conferences, as needed. * Mentor other associates as needed or required * Performs other tasks as assigned. * Conducts job responsibilities in accordance with the standards set out in the Company's Code of Business Conduct and Ethics, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards. * To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Responsibilities * Clinical Assessment and Documentation: * Conduct initial and ongoing clinical assessments of patients, inquiring about vital signs, gathering all information regarding pt condition or issue, prioritizing pt issues, assigning risk category and documenting all patient care data in the Electronic Health Record (EHR). * Contribute to the development and updating of individualized care plans in collaboration with care team and providers. * Maintain productivity metrics. * Care Coordination and Patient Education: * Coordinate with providers, family members, and caregivers to facilitate the implementation of care plans, ensuring all parties are informed and engaged in patient care. * Provide education to patients and caregivers on treatment plans, medication management, and other care needs to improve patient outcomes and adherence. * Medication and Treatment Management: * Handle medication-related inquiries, process prior authorizations, and document all interactions with patients regarding medication adjustments. * Review and process clinical orders as needed, ensuring compliance with provider instructions and patient needs. * Compliance and Quality Assurance: * Ensure all clinical documentation meets regulatory and compliance standards, adhering to HIPAA and company policies. * Participate in regular care management meetings and quality assurance checks to maintain high standards of patient care. * Collaboration and Communication: * Work closely with Care Facilitators for scheduling support and non-clinical inquiries to focus on core clinical duties. * Participate in team meetings and provide insights on patient care processes, identifying areas for clinical improvement Qualifications * Associate degree preferred * Prior experience in a patient facing capacity required * Familiarity with Care Management preferred * Experience with Transitional Care Management strongly preferred * Minimum of 3-years of experience in geriatric or complex patient care * Experience working with Electronic Health Records (EHR) software and a Heath Information Exchange * Licensed Practical Nurse (LPN) with an active license; compact license preferred. * Excellent communication skills * Comfortable assessing status of patient remotely, active listener, motivational interviewing skills * Ability to recognize condition specific symptoms * Organized, thorough documentation skills * Solid time management skills * Ability to work independently as well as part of clinical care team * Ability to develop and build trusting relationships with patients and partners * Able to use Microsoft Excel for tracking and reporting About our Line of Business Abode Care Partners, an affiliate of BrightSpring Health Services, is a leading provider of integrated medical services, caring for individuals from post-hospitalization to home in various settings ranging from skilled nursing facilities, assisted living, independent living, group homes, and private homes.
