Accurately accounts for the whereabouts of each client by conducting and documenting facility whereabouts three (3) times per shift in accordance with Agency policy and procedures.
Maintains an active presence on the floor (common and dorm areas) by conducting frequent circulations of the facility at a minimum 30 minute interval. Checks for unsafe conditions and facility and dorm cleanliness. Documents and corrects conditions/takes steps to correct conditions as required.
Completes scheduled activities including, but not limited to: intakes, random alco-sensor tests, urine drug screens, dorm searches, pat downs, and emergency drills (fire, tornado, etc.) in accordance with established policies and procedures.
Conducts alcohol testing and collects urine specimens in accordance with Agency policy and procedure.
Conducts client intakes. Ensures that all forms are filled out accurately and completely (e.g., are signed by the client and the staff member). Ensures that all information, including PREA results, is documented in ORION under client profile.
QUALIFICATIONS:
High School diploma or equivalent required. Associate's Degree in human services field preferred. Relevant job experience may be substituted for preferred college degree. Must possess a valid Ohio driver's license and a driving record that does not preclude the employee from being covered by Agency liability insurance. Must have the cognitive skills needed to complete tasks, including abilities such as learning, remembering, focusing, categorizing, and integrating information for decision making, problem solving, and comprehending. Has the ability to perform under stress and/or in emergencies. Working on a ‘one to one' basis with the residents by teaching independent living skills and programs.

Works collaboratively with Community Support Treatment Workers and staff to support the progress of our clients.

Position Requirements:
Associate's degree in social work or related field and 2-3 years related experience or training preferred. High School diploma/GED required at minimum.
2+ years of working with clients in a recovery capacity preferred.
Must have an acceptable driving record (4 points or less), reliable transportation, and carry automobile insurance with minimum liability limits of $100,000/$300,000 and property damage of $50,000.
Experience with trauma informed care, motivational support, stage wise treatment, substance use, crisis de-escalation highly preferred.

PRIMARY DUTIES AND RESPONSIBILITES
Facilitate resident medical and other outside appointments, including but not limited to communicating appointments to residents and appropriate staff, and transporting residents to their appointments.

Ensure that all physical and hygiene needs are met for residents.
Process and submit critical incident reports by THE END OF EACH SHIFT.

Supervises and supports assigned residential behavioral health staff.
Delegates and manages responsibilities from shift to shift.
Carries out individual performance inventory semi-annually as assigned.
Expedites referrals and intakes for service access.
Supports and sustains connections with community partners.
Troubleshooting scheduling issues and needs to be presented to the program manager.
Conducts weekly care team meetings to monitor progress and discuss resident care plans.
Provides onboarding and training for all new staff and reviews as needed with all employees.
Work directly with clinical staff regarding daily scheduling issues and specific treatment plans for residents.
Schedules and facilitates special activities for residents.
Monitors the floor and deals with any difficult situations that arise daily, as well as supports staff.
Communicates with parents/families.
Be available to cover groups when needed.
Oversees and facilitates family days.
Acts as primary contact for both clinical and non-clinical program emergencies during shift.
Ensures safety in the community.
Minimum Requirements:
Minimum three years of directly relevant experience or a Bachelor's degree.
High School Diploma or GED.
Supervisory experience preferred.
Demonstrated a high degree of cultural awareness.
Experience with multi-need individuals and families.
Broad knowledge of facility-based service systems.
Licensed and able to operate a motor vehicle with an appropriate level of insurance coverage.
Willing to participate in and lead cross-systems team-building activities.
Able to effectively communicate through verbal/written expression.
Must be able to operate in an Internet-based, automated office environment.
