Responsibilities

Maintain client safety and security including assistance with emergency evacuation and other special needs.
Maintain Shelter records including logs, rosters, inventory lists, supply orders, maintenance requests, etc.
Conduct monitored drug / alcohol testing.
Attend all staff meetings / trainings as directed.
Participate in all other duties and activities as requested or assigned.

Community Residence Aides (CRAs) in our Residential Department work within group homes and apartment settings to provide a wide variety of person-centered, trauma informed supports and services to adults with a mental health diagnosis. CRAs will provide support with daily living activities and socialization; while modeling, teaching, and advocating for them to live at their highest level of independence. CRAs are responsible for the supervision and safety of those residing at their site, overseeing medication self-administration, medical appointment participation, and household chores and cleaning. Paid training is provided. Sites are staffed 24 hours a day, 7 days a week. Residence locations are on the East Side of Buffalo.
CRAs working 3pm-11pm Monday through Friday will receive a shift differential of $1.50 per hour. CRAs working 11pm-7am Monday through Friday will receive a shift differential of $2.00 per hour. The Personal Care Aide (PCA) is responsible for providing assistance with activities of daily living to assigned residents in accordance with all applicable laws and regulations as well as our community's established policies and procedures.
Essential Functions
* Assist residents with all activities of daily living including bathing, dressing, grooming, toileting, and eating
* Observe residents' conditions, and report changes to supervisor.
* Answer residents' call lights in a timely fashion.
* Provide residents with help walking, exercising, and moving in and out of bed.
* Prepare, serve, and collect meal trays.
* Clean rooms and change linens.
* Deliver messages, documents to residents or staff.

* CPR certification upon hire or obtained during orientation.

Participate in planning and conducting of individual, small and large group activities. Assist in providing good communication between employees of all levels, residents, their families, support personnel, government agencies/personnel, and the public to ensure the needs and best interest of the residents, community and facility are met to the extent possible.
Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
Assist in development of monthly activity calendar and maintaining attendance records.
Assist the Quality Assurance and Assessment Committee in developing and implementing appropriate plans of action to correct identified quality deficiencies as requested by Activities Director.
Participate in discharge planning, development and implementation of activity care plans and resident assessments.
Assist in arranging transportation for residents when necessary including, but not limited to, resident outings, to/from appointments, or for discharge.
Assists in developing a monthly activity schedule for residents, which include resident outings, regularly scheduled group activities, and in room activities for bed bound or isolated residents.
Encourage residents to participate in self initiated activities including hobbies, crafts, and reading Provide materials as necessary including reading materials in Braille or audio books as appropriate.
Assist with assessment documentation, quarterly progress notes as assigned by the Activity Director.
Keep Activity Department clean, orderly and secured.
Supervisory Requirements This position has no supervisory responsibilities.
Qualification Education and/or Experience High school diploma or equivalent.
Preferable one year experience in a long term care facility.
Language Skills Ability to read technical procedures.
Ability to read and comprehend policy and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Mathematical Skills Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Reasoning Ability Ability to solve practical problems.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities
A. DIRECT SERVICE RESPONSIBILITIES:
Provide teaching, supervision, and support to residents in areas of independent living.
Observe, monitor, and document resident behaviors as well as their progress in accordance with agency standards.
Assist in the preparation of nutritious meals, including food preparation when needed.
Assist in the implementation and supervision of recreational activities.
Complete household chores assigned by supervisor to maintain cleanliness and safety standards.
Supervise and document resident medications in accordance with agency policies.
Perform any other duties necessary for the effective and efficient operation of the community residence.
B. ADMINISTRATIVE RESPONSIBILITIES:
1. Attend staff meetings and trainings as required by supervisor and work as a member of the residential team for each resident. Give relevant input for treatment team meetings, staff meetings, utilization reviews, and any other meetings.
2. Observe all safety rules and regulations for self and others.

Education: High School Diploma
Experience: Human services experience preferred
Other: Driving record that meets agency's Clean Driving Record Policy.
Work Environment
All per diem positions are required to work at least 4 shifts per month.
