Job Announcement

With more than 25,000 acres of parkland, nine REC Centers, eight golf courses, three lakefront parks, and more, FCPA is an exciting organization to be a part of and there are many opportunities for staff to make a difference at every level.
Under minimal supervision assists in the day to-day operations of services offered in the Park Operations branch. Primary point of contact for Park Operations maintenance phone line and email. Responds directly to a high volume of customer inquiries and customers over the telephone and by email. Assists in the divisions hiring, interviewing, and onboarding processes. Communicates daily with Park Authority staff to research and resolve inquiries and maintenance requests. Manages or coordinates branch programs such as agency motor pool, Adopt-A-Field, Vegetative Planting Requests, etc. Enters, validates, and updates asset information management system. Manages the Park Operations office. Successful applicants will be self-motivated and have excellent interpersonal communication skills to mitigate user concerns and promote agency services.
Position reports to 12055 Government Center Pkwy, Fairfax, VA 22035 3 Days a week, teleworks 2 days a week.

The ResidentialAssistant supports the individuals living in Linwood's residences and assists in implementing each resident's Individual Plan (IP) and Behavior Plan (BP). Essential Duties and Responsibilities:
Participates in developing the IP and behavior plans for each individual in the house and implements and documents the portion of the plan related to the individual in the residential program.
Provides guidance to the individuals in the house to ensure the following:
The house, the appliances, the vehicles, and the equipment are safe, operational, and in good working order.
The individuals are engaged in meaningful activities that address the outcomes in their IPs.
Supervises and assists the individuals in the house with menus, food preparation, housekeeping, personal care, laundry, and other activities of daily living.
Plans and coordinates appropriate leisure, social, and daily activities in the residence and the community.
Provides transportation of the individuals to and from their destinations. These activities may include events, shopping trips and medical, dental, and mental health appointments.
Acts as an agent of Linwood by exchanging appropriate and accurate information with the healthcare professional during medical, dental, and mental health appointments and interact with the pharmacist to obtain medications.
The nursing staff commissioned by Linwood shall train the residentialassistant and carry out duties as a certified medication technician, delegated by the nurses as prescribed by the Board of Nursing and DDA.
Administers correct and timely medication (oral, topical, nebulizers, bowel regimens) consistent with the Physician Medical Order Form (PMOF) and the Medication Administration Record (MAR) and ensures three-way checks.
Assumes responsibility for the individuals in the house during the day when they cannot attend the day program (e.g., for illness, inclement weather, closings, etc.). The supervisor will try to arrange coverage; however, the final responsibility rests on the residential staff.
Meets deadlines and completes all necessary paperwork in conjunction with the House Manager. This includes quarterly reports, monthly reports, fire drill records, attendance records, weight charts, house receipts, daily logbooks, etc.
Attends and participates in clinical meetings and medication reviews as directed.
Accounts for money that belongs to the individuals per Linwood's policy and procedure regarding the Protection of Finances and Personal Property.
Uses technology for the completion of specified job duties.
Learns and properly utilizes Linwood's Data Management System (CIMS) to maintain accurate records and permit accurate billing with DDA.”

The opportunity:
The ResidentialAssistant is responsible for providing support to the individuals living in Linwood's residences. The ResidentialAssistantassists in the implementation of each resident's Individual Plan (IP) and Behavior Plan (BP). This support includes facilitating the resident's achievement of personalized goals that are tailored to his/her needs and ability. The ResidentialAssistant will work cooperatively with other assistants in the home, the House Manager, Senior Residential Managers and other staff.
Essential Responsibilities:
Participation in the development of the IP and Behavior Plans for each of the individuals in the house and implementing and documenting the portion of the plan(s) related to the individual in the residential program.
Providing guidance to the individuals in the house to ensure the following:
That the house, the appliances, the vehicles and the equipment are safe, operational and in good working order.
That the individuals are engaged in meaningful activities that address the outcomes in their IPs.
That the fundamental rights of each adult in the house are protected.
Supervising and assisting the individuals in the house with menus, food preparation, housekeeping, personal care, laundry and other activities of daily living.
Planning and coordinating appropriate leisure, social and daily living activities in the residence and in the community. This includes transporting the individuals to and from their destinations. These activities also include shopping trips and medical, dental and mental health appointments.
During medical, dental and mental health appointments, the ResidentialAssistant is an agent of Linwood and will exchange appropriate and accurate information with the healthcare professional and interact with the pharmacist to obtain medications. The ResidentialAssistant shall be trained by the nursing staff commissioned by Linwood and will carry out duties as a Certified Medication Technician delegated by the nurses as prescribed by the Board of Nursing and DDA.
Assuming responsibility for the individuals in the house during the day when they are unable to attend the day program (e.g. for illness, inclement weather, closings, etc.). The supervisor will make every effort to arrange coverage, however, the final responsibility rests on the residential staff.
Meeting deadlines and completing all necessary paperwork, in conjunction with the House Manager. This includes quarterly reports, monthly reports, fire drill records, attendance records, weight charts, house receipts, daily log book, etc.
When directed by the House Manager, the ResidentialAssistant will be required to attend and participate in clinical meetings and medication reviews.
Accounting for money that belongs to the individuals per Linwood's policy and procedure regarding the Protection of Finances and Personal Property.

The Opportunity:
The ResidentialAssistant is responsible for providing support to the individuals living in Linwood's residences. The ResidentialAssistantassists in the implementation of each resident's Individual Plan (IP) and Behavior Plan (BP). This support includes facilitating the resident's achievement of personalized goals that are tailored to his/her needs and ability. The ResidentialAssistant will work cooperatively with other assistants in the home, the House Manager, Senior Residential Managers and other staff.
Essential Duties and Responsibilities:
Participation in the development of the IP and Behavior Plans for each of the individuals in the house and implementing and documenting the portion of the plan(s) related to the individual in the residential program.
Providing guidance to the individuals in the house to ensure the following:
That the house, the appliances, the vehicles and the equipment are safe, operational and in good working order.
That the individuals are engaged in meaningful activities that address the outcomes in their IPs.
That the fundamental rights of each adult in the house are protected.
Supervising and assisting the individuals in the house with menus, food preparation, housekeeping, personal care, laundry and other activities of daily living.
Planning and coordinating appropriate leisure, social and daily living activities in the residence and in the community. This includes transporting the individuals to and from their destinations. These activities also include shopping trips and medical, dental and mental health appointments.
During medical, dental and mental health appointments, the ResidentialAssistant is an agent of Linwood and will exchange appropriate and accurate information with the healthcare professional and interact with the pharmacist to obtain medications. The ResidentialAssistant shall be trained by the nursing staff commissioned by Linwood and will carry out duties as a Certified Medication Technician delegated by the nurses as prescribed by the Board of Nursing and DDA.
Assuming responsibility for the individuals in the house during the day when they are unable to attend the day program (e.g. for illness, inclement weather, closings, etc.). The supervisor will make every effort to arrange coverage, however, the final responsibility rests on the residential staff.
Meeting deadlines and completing all necessary paperwork, in conjunction with the House Manager. This includes quarterly reports, monthly reports, fire drill records, attendance records, weight charts, house receipts, daily log book, etc.
When directed by the House Manager, the ResidentialAssistant will be required to attend and participate in clinical meetings and medication reviews.
Accounting for money that belongs to the individuals per Linwood's policy and procedure regarding the Protection of Finances and Personal Property.
You Must:
Be at least 18 years old and have a high school diploma or equivalent.

Possess effective oral and written communication skills.

RESPONSIBILITIES/ACCOUNTABILITIES
Provide direct care to residents and contribute to continuity of care and effective communication by reporting care provided and changes in the resident's condition to the Resident Care Director immediately. May participate in routine activities of resident's care activities, including daily living.
Assistingresidents with lifts, moves, and transports, using proper body mechanics or lifting devices for accident prevention.
Responds to inquiries relating to requests from residents, visitors, and other personnel promptly and courteously within given time frames and established policy.
Is knowledgeable of the individualized care plan for residents and provides support to the residents according to their care plan. Contributes to the care planning process by providing the charge nurse or other care planning staff with specific information and observations of the residents' needs and preferences.
Communicates and interacts effectively and tactfully with the residents, visitors, families, peers, and supervisors promoting a homelike environment.
Fully understands all aspects of residents' rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents' rights.
Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials. Also providing care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas.
Arrive to work at the scheduled time.
Perform other related duties as required.
Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
Report any occupational exposures to blood, body fluids, infectious materials, and or hazardous chemicals in accordance with facility policy.
Report work related injuries and illnesses immediately to your supervisor.
As a condition of employment, complete all assigned training and skills competency.
Participate in all life safety and emergency drills and trainings.
Fulfill responsibilities as assigned during implementation or activation of the facility's emergency plan.
