Location: Falls Church, VA Pay: Between $80,000 and $90,000 per year based on experience About the Position This position is primarily responsible for providing clinical social work services to residents of Assisted Living and The Terrace (memory support) and their families to promote general social and emotional well-being. This position will also provide back-up support to the social workers for Residential Living and the Health Care Center, as needed. Key Duties * Interviews potential Assisted Living residents from GHBC and the community to complete a social history and cognitive assessment in determining level of care. * Offers direct social work interventions to residents and their families and provides appropriate measures for helping residents in their personal adjustment, emotional and social well-being and accessing of appropriate facility and community referrals and resources. * Accomplishes certification by the Virginia Department of Social Services (VDSS) to participate in the completion of the Universal Assessment Instrument (UAI) and development of the Individualized Service Plan (ISP). * Participates in the weekly interdisciplinary team meeting and collaborates with interdisciplinary team members to assist with Assisted Living admission, transfer and discharge planning. * Provides quality resident care by coordinating and facilitating conferences with families/powers of attorney and other interested parties to address problems of adjustment, treatment, and caregiving concerns. Job Requirements * An MSW degree from an accredited university; prefer a licensed clinical social worker (LCSW). * Must possess computer skills; prefer experience with electronic medical records. * Prefer to have at least three (3) years experience in social work with at least one (1) year in work with the geriatric population. * Must possess ability to deal tactfully and effectively with residents, staff, family members and the public A sampling of our many benefits!
