Build Meaningful Connections: * Engage residents in social activities and one-on-one interactions. * Foster a positive, respectful environment with residents, families, and team members. * Encourage participation in group activities to promote a sense of community. Promote Safety & Mobility: * Help transport residents to appointments and events. * Safely assist with lifting, turning, and positioning residents. Monitor & Communicate Changes: * Follow individualized care plans and provide updates as needed. * Observe and report changes in residents' conditions to supervisors. * Record vital signs and care details accurately in electronic health records.
