POSITION SUMMARY Our Resident Lifestyle Assistant (Activity Assistant) supports the Resident Lifestyle Director in designing, creating, initiating, facilitating, and coordinating various daily life enrichment activities for our active residents, aimed to promote their physical, mental, emotional, and social well-being. KEY RESPONSIBILITIES Engage residents one-on-one and in small and large group settings. Lead or assist with group and one-on-one social, religious, educational, indoor, and outdoor activities, exercise, and outings. Facilitate games, conversations, exercise classes, art and music, trips, and special events. Support the coordination of the monthly newsletter. Encourage resident participation in activities and document outcomes when required. Review goals and progress notes. Assist in decorating for special events, holidays, and seasonal celebrations. Support the coordination of external trips, including reservations, and ensure resident safety during outings. Assist the Resident Lifestyle Director as necessary. Requirements: CANDIDATE QUALIFICATIONS Education: High School Diploma or equivalent. Experience, Competencies, and Skills: At least 6-12 months of relevant experience working with seniors, leading life enrichment activities is preferred. Working knowledge of various computer systems, including Word, Excel, and Outlook. Ability to work independently and follow scheduled plans. Ability to maintaining composure in stressful situations and commitment to always treating our residents with respect and dignity. Friendly, assertive, professional, outgoing, multitasked, and organized. Excellent communication skills. Availability to work flexible shifts, including weekends and holidays.

    Job Description The Community Resource Assistant supports AllThrive 365's community service programs to attain contractual program objectives and goals by providing direct assistance and case management services to vulnerable populations using resources from various funding. The Community Resource Assistant fosters self-sufficiency within the community by identifying resources for clients, assessing their needs, and connecting them to local support systems. This role is designed to bridge the gap between families and the resources they need to thrive, including education, healthcare, social services, and community programs. The ideal candidate is passionate about child welfare, support with community outreach, and experienced in working with children and families from diverse backgrounds. Essential Functions: % of Time Community Resources Center Support Implement strategies is to provide a welcoming, safe, and accessible community hub that provides flexible, multi-generational, family-focused, and culturally responsive information, resources, and services covering a wide range of topics. Assist with identifying and connecting children and families with appropriate community resources, including housing assistance, food programs, healthcare, educational support, and mental health services. Assist in designing, implementing, and evaluating community programs that support the development, safety, and well-being of children and families. Assist with organizing and facilitating educational workshops for parents and caregivers on topics like child development, healthcare, academic support, and family financial planning. Maintain accurate records of all client interactions, services provided, and outcomes. Regularly report on program activities, successes, and areas for improvement. Maintain accurate, timely records of resource center activities and client demographics, adhering to all reporting requirements for funding sources and program evaluation. Support outreach initiatives tailored to client needs, and work to elevate AllThrive 365's presence and impact in the community, supporting growth in community resource center and other essential services through relationship-building. 50% Intake, Assessments and Case Management First point of contact for applicant screening of the programs and referrals Performs client intake procedures for all referral and walk-in clients by completing telephonic intakes, eligibility screenings, document collection, applications, and referrals Performs assessment of client needs and determines eligibility for programs and services Communicates in person or via email or telephone regarding qualifications, follow-ups or requirements Provides case management in a confidential, timely, effective, efficient, and client-centered manner Coordinates and provides services such as budgeting and finances, employment services and other necessary support systems that will increase the person's ability change, be self-sufficient and/or improve their quality of life Provides referral services for clients whose needs are outside their scope of practice and/or not available at the program site Completes and compiles demographic information for funding sources 10% Custodian of Records Processes and maintains application using electronic records system Maintains client files in accordance with funding sources regulations, contract requirements and AllThrive 365 procedures, with special emphasis on client confidentiality 10% Quality & Compliance With a strong focus on client satisfaction, ensures compliance with all AllThrive 365 policies, procedures, and practices, and other contract and licensure requirements, and funding sources; and applies knowledge to the work in accordance with quality standards Records and compiles data as a part of the program's quality and performance review Attends and actively participates in initial and on-going training and development programs and opportunities, weekly unit meetings all staff meetings, and demonstrates learned concepts on the job Participates in assigned work teams, as requested and appropriate Engages in continuous improvement efforts, as assigned, and strives to continuously improve the quality of care and self Communicates daily and as necessary with colleagues, supervisor, and leadership 10% Program Support * Supports our programing to fill in gaps and cover for staff or volunteers including, but not limited to delivering meals, providing rides, working food bank, covering front desk. *Their duties may vary depending on the specific program, but common responsibilities include: Entering participant information, updating records, and maintaining databases. POSITION SUMMARY Our Resident Lifestyle Assistant (Activity Assistant) supports the Resident Lifestyle Director in designing, creating, initiating, facilitating, and coordinating various daily life enrichment activities for our active residents, aimed to promote their physical, mental, emotional, and social well-being. Requirements CANDIDATE QUALIFICATIONS Education: High School Diploma or equivalent.
