POSITION SUMMARY Our Resident Lifestyle Assistant (Activity Assistant) supports the Resident Lifestyle Director in designing, creating, initiating, facilitating, and coordinating various daily life enrichment activities for our active residents, aimed at promoting their physical, mental, emotional, and social well-being. KEY RESPONSIBILITIES Engage residents one-on-one and in small and large group settings. Lead or assist with group and one-on-one social, religious, educational, indoor, and outdoor activities, exercise, and outings. Facilitate games, conversations, exercise classes, art and music, trips, and special events. Support the coordination of the monthly newsletter. Encourage resident participation in activities and document outcomes when required. Review goals and progress notes. Assist in decorating for special events, holidays, and seasonal celebrations. Support the coordination of external trips, including reservations, and ensure resident safety during outings. Assist the Resident Lifestyle Director as necessary. Requirements CANDIDATE QUALIFICATIONS Education: High School Diploma or equivalent. Experience, Competencies, and Skills: At least 6-12 months of relevant experience working with seniors, leading life enrichment activities, is preferred. Working knowledge of various computer systems, including Word, Excel, and Outlook. Ability to work independently and follow scheduled plans. Ability to maintain composure in stressful situations and commitment to always treating our residents with respect and dignity. Friendly, assertive, professional, outgoing, multitasked, and organized. Excellent communication skills. Availability to work flexible shifts, including weekends and holidays. WORK SCHEDULE * Full-Time / 40 hours per week * Day Shifts HIGHLIGHTS * Partner with Social Workers (SW), Nurse Care Coordinators (NCCs), patients, their families and the health care team to address patients' progression of care * Assist the SW and NCC team in the coordination of the discharge planning process for patients PRIMARY JOB RESPONSIBILITIES * Facilitate the placement of patients in long term care facilities based on data obtained from clinical staff. Assist the SW/NCC in arranging for home care needs, such as referrals to home health, hospice, in-home services or procurement of home Wound VAC. * Assist with safe transition of patient to home or other facilities, including locating outpatient non-institutional settings (respite, shelter, housing, etc.) * Assist the patient/family with emergency needs for resources such as emergency funds, food vouchers, and transportation subsidies. * Accept and act on referrals from SW/NCCs in a timely manner, keeping the SW/NCC informed of progress on the case, documenting in the EMR per department standards, and ensuring that handoffs to all parties are clear and timely. * Assist the SW/NCCs in identifying, researching, explaining and arranging resources for families. Resources include, but are not limited to, transportation, housing, financial assistance, and safety net programs. * Under the direction of the SW/NCC, prepare letters and documents to assist families in obtaining benefits. Copy/fax/mail documents as required. Perform other clerical duties as required. * Provide notary public services to patients and staff. Strong candidates will have a minimum 3 years healthcare support services experience. Excellent written and verbal communication skills are a must, as well as the ability to organize and prioritize work and handle a fluctuating workload. Responsibilities Greet customer warmly and assess reason for visit The Assistant Service Advisor/Writer is responsible for the entire customer service experience until vehicle is turned back over to the customer Generate estimates, suggest services and advise of any current promotional products/services Advise customers on the routine care of their vehicles and the value of maintenance in accordance with the manufacturer's specifications Communicate to the customer on targeted completion time.
