The Day Program DSP is an active and integral member of the habilitation team at 360 Total Care, dedicated to supporting and enhancing the lives of participants with developmental disabilities.
This position involves extensive community interaction as specialists accompany participants to various activities and events within and around the city, advocating for, and assisting them in their social and personal development.
This role requires safely transporting 1 or more individuals with developmental disabilities to and from the Day Habilitation Center, residential homes, community events, and other scheduled activities. This role requires strict adherence to Ohio Department of Developmental Disabilities transportation regulations and organizational policies.
Schedule:
Core work hours between 7AM - 4PM Monday through Friday based on transportation routes. (Up to 40 hours weekly.)

Key Responsibilities:
Community Integration:
Spend approximately 5 hours per day in community settings with a group of participants, facilitating and supervising daily outdoor activities.
Engage participants in a variety of community activities, including volunteering, attending fun events, and fostering social relationships.
Advocate for participants' needs and rights within the community, ensuring their inclusion and respect.
Behavioral and Support Ratios:
Manage groups with a typical ratio of 4:1, adjusting to 1:1 as necessary for participants with high behavioral needs.
Transportation Services:
Safely transport participants to and from various locations in a timely and efficient manner.
Assist passengers in boarding and exiting vehicles, ensuring safe transfer and secure travel.
Operate a range of vehicles, including vans and minibuses equipped with wheelchair lifts and other accessibility features.
Provide assistance and support to passengers as required, including securing wheelchairs and other safety devices.
Maintain a respectful and supportive interaction with all passengers, ensuring a positive and safe travel experience.
Vehicle Maintenance and Safety Compliance:
Conduct pre-trip and post-trip vehicle inspections to ensure operational safety.
Maintain cleanliness and general upkeep of the vehicle.
Report any vehicle maintenance needs or safety concerns to the Day Habilitation Program Manager immediately.
Documentation and Compliance:
Complete all required daily documentation, including participant activities, progress, and any incidents, in compliance with organizational and state regulations.
Maintain accurate travel logs and documentation of vehicle usage.
Ensure all transportation activities are logged according to DODD and organizational requirements.
Report any incidents or accidents immediately, following established protocols.
Training and Compliance:
Participate in required training sessions, including First Aid, CPR, and specialized transportation safety courses.
Stay updated with all DODD regulations and guidelines pertaining to the role.

Essential Duties
Provide 5-Star care to our amazing residents
Bathe, shave, dress and undress, care for hair, finger nails, eyes and ears (cleaning eyeglasses, etc.), perform daily dental care, perineal care, as well as any other customary daily activities of living for residents.
Lift, turn, move, position, transfer and transport residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, vehicles, etc.; assist resident with walking with or without self-help devices.
Respond to resident calls promptly. Ensure call cords are within reach of residents. Ensure residents who are unable to call for help are checked frequently.
Assist resident with bowel and bladder functions; take to bathroom, offer bedpan/urinal, etc. Collect specimens as instructed.
Keep incontinent residents clean and dry. Check and report bowel movements and character of stools as instructed. Change clothing, linens, etc. when it becomes wet or soiled.
Change bed linens, make beds (occupied and unoccupied); keep linens tight to avoid wrinkles from forming under the resident; provide extra covers as requested.
Routinely checks on residents to ensure personal care needs are being met. Turn bedfast residents at least every two (2) hours.
Perform restorative, rehabilitative and special treatments procedures as instructed. Applies slings, ace bandages, and braces as instructed. Provide Range of Motion exercises.
Assist resident in preparing for activity and social programs, meals, appointments, medical tests, etc.
Prepare residents for meals and perform after meal care; take to bathroom, wash hands and face, transport to and from, brush teeth, clean dentures, etc.
Observe and record intake of food and fluids; report changes in eating habits.
Serve food trays, assist with feeding as indicated (cutting foods, feeding, orienting to food items and placement and temperatures). Keep water pitchers clean and filled with fresh water and within easy reach of the resident. Serve snacks.
Weigh and measure residents as instructed. Measure and record temperatures, pulse, and respirations (TPRs), as instructed. Observe and report symptoms, reactions, and changes, such as the presence of pressure areas, bedsores, and skin tears.
Participate in and receive nursing reports at shift changes. Assist with carrying out the service plan document (AL) or care plan document (HC), through the point of care application.

Friends Care Community in Yellow Springs, Ohio is a not-for-profit Life Care Community consisting of 59 SNF beds, 20 AL beds and 22 IL homes. We put our Mission of care for our residents before anything else. We are seeking an Activities Assistant to join our team who shares our same commitment to our residents!
Primary Responsibilities:
Facilitate activities as directed by the Activity Director (i.e., setting up necessary equipment and supplies, escorting residents to and from activities area and providing instruction).
Supervise and assist residents engaged in scheduled activities as necessary.
Make routine visits to in-room residents and assist them by writing letters, running errands, making appointments and providing assistance with crafts projects, etc., as necessary.
Observe and document resident attendance, mood, behavior and degree of involvement in facility activities.
Interview residents or family members to obtain activities information.
Assist in developing activities-related interventions in resident care plans as appropriate.
Encourage the resident/family to participate in the development and review of his/her plan of care.
Develop and maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruptions.
Report all incidents/accidents and safe/unsafe conditions immediately.
Follow all established safety and ergonomics policies and procedures.
Assist in preparing activities rooms/areas for scheduled activity programs.
Assist in cleaning up activities area after completing activity functions.
Operate activities equipment safely and efficiently.
Requirements:
Must have experience in a patient activities program in a health care setting.
Must have a valid driver's license.
