Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Mental Health Counselor in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.

The Academic Interventionist is responsible for working directly with students to provide tutoring, coaching, goal-setting, and academic case management to achieve measurable improvements in academic performance.

Instructional Intervention

Education and Experience

POSITION OVERVIEW
Under supervision of the Clinical Supervisor the CASAC-T Counselor will provide counseling services in integrated outpatient program. The CASAC-T Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC-T Counselor is also responsible for referrals and coordination of care in consultation with other professionals.
KEY ESSENTIAL FUNCTONS
Intervention and employing a range of treatment modalities to include person centered evidence-based treatment approaches to include but not limited to motivational interviewing, stages of change, trauma informed care, anger management, medication assisted treatment, dialectical behavioral therapy, cognitive behavioral therapy and contingency management.
Ability to perform SUD and mental health screenings and intakes, orientation, SUD assessment, SUD diagnostic Impression, level of care as determined by LOCADTR, treatment planning, individual/group counseling, family interventions, psychoeducation, case management, crisis intervention, client education, referral, reporting and recordkeeping, care coordination and administrative supervision with regular supervision.
Participates in supervision regularly.
Develops and conducts group and individual counseling on a regular scheduled basis to caseload clients within the integrated outpatient program.
Obtain random observed urine samples according to established protocol.
Orients clients about program goals, objectives, rules and regulations, client's obligations, right's and program operations
Knowledge of the obligation to engage in prevention as well as treatment techniques.
Maintains and updates referral services and resources that may be useful to our client population; makes referral when necessary and appropriate.
Ability to demonstrate proficiency in English including the ability to speak, write, comprehend aurally, and read at a level necessary to perform as a CASAC Counselor.

The Pride Center of the Capital Region is hiring a Mental Health Counselor! We are looking for an experienced Mental Health Counselor that's committed to helping the LGBTQ+ community to join us and expand the impact of the Pride Center! Position Summary: The Mental Health Counselor is responsible for providing affirming mental health counseling and case management services, particularly for LGBTQ+ youth and young adults, with an emphasis on transgender, gender non-conforming, and nonbinary (TGNCNB) individuals. The Mental Health Counselor collects information about clients through engagement, observations, and assessment and guides clients in the development of skills and strategies to deal with their challenges. This role involves both direct client services and the supervision of Master of Social Work (MSW) and Bachelor's of Social Work (BSW) interns. The Mental Health Counselor will contribute to expanding the Pride Center's mental health services to rural areas, ensuring accessible, culturally competent care for LGBTQ+ individuals.
Responsibilities: Counseling and Case Management:
Provide individual and family counseling services, primarily to LGBTQ+ youth and young adults to assist in overcoming dependencies, adjusting to life, and making changes.
Manage a caseload of clients, including intake assessments, treatment planning, and ongoing support.
Supervise MSW/BSW interns, providing guidance, mentorship, and evaluation to support their professional development.
Offer crisis intervention services, including safety planning and referrals to appropriate resources.
Maintain timely and accurate documentation of client interactions in accordance with state and federal laws.
Program Operations:
Collaborate with the Mental Health Director to expand mental health services to underserved rural counties.
Assist in the planning and implementation of on-site and telehealth services in partnership with community organizations.
Participate in the recruitment and supervision of MSW interns, ensuring they deliver affirming, culturally competent care.
Contribute to the development and delivery of mental health education and training for staff, interns, and community partners.
Ensure compliance with all clinical and ethical standards for providing LGBTQ+ affirming mental health services.
Data and Reporting:
Maintain client records and program data to track service hours, client demographics, and outcomes.
Report regularly to the Mental Health Director on program effectiveness, client needs, and service utilization.
Assist in preparing reports for grants and other funding sources as required.
Collaboration and Leadership:
Work with community partners to promote mental health services, ensuring services are accessible and responsive to the unique needs of LGBTQ+ individuals in rural areas.
Serve as a role model for MSW/BSW interns, promoting a supportive and inclusive environment
Participate in regular staff meetings and supervision sessions to discuss client progress and address any concerns.

Qualifications:
Licensure: Provisional Counselor License required, LCSW preferred.
Education: Master's Degree in Social Work, Seminar in Field Instruction (SIFI), certification preferred
Experience: Experience/Education in an LGBTQ+ affirming setting preferred. Experience supervising MSW/BSW interns is a plus.
Skills: Excellent counseling, case management, and supervision skills. Familiarity with LGBTQ+ mental health issues and the unique challenges faced by TGNCNB individuals is essential.
Communication: Strong written and verbal communication skills, with the ability to work both independently and as part of a collaborative team.
Ability to Travel: The role is being built out to support enhanced work with the rural communities in our program areas, as such, this role requires a staff member with a vehicle and ability to travel. Mileage expenses are reimbursed at the federal rate.
Background check: Must be able to pass a background check and work with vulnerable populations including youths.
