For surgery evaluations runs a Criminal Background Check (CBC). -Contacts client to over the ISO and/or CBC with client to confirm the information and/or obtain any pre-ret treatment regarding any previous accidents. Adds a note regarding the ISO/CBC review. -CM will obtain medical provider full contact information, prior attorney information and provide this to their respective RC for cases where there are pre-existing accidents as they will request the records. Description: Residential Advisor - Substitute On-Call Reports To: Senior Residential Advisor Department: Social Development FLSA STATUS: Non-Exempt This position is an on-call position and work is on an as needed basis. Responsible for a paraprofessional level of student support and performance duties necessary for an orderly operation of the Dormitory Program. Must be available to work any shift and day of the week due to a 24/7 operation. Administration Procedures • Provides substitute and/or call-in services of a residential advisor. • Ensures orderly, clean living conditions by scheduling and supervising, and participating in dorm clean up and submitting repair requests. • Provides supervision for the cafeteria, laundry, special activities (on and off Center) and other areas of the program as scheduled or assigned. • Identifies and assists students in handling special problems and conducts dormitory meetings. • Maintains individual records for each student that contain, at a minimum, basic identifying information, including emergency contacts, and written parental consent (minors) for weekend passes to approved destinations. Directs the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations and Company policies and procedures to assist each resident and family adjust to placement, illness, and plan of care so as to attain the highest practicable level of functioning. Duties and Responsibilities Assists in interviewing and obtaining information from residents and families to assist Social Services Director or Social Services consultant in development of psychosocial history, assessment and care plan upon admission, quarterly and upon change of condition for social service needs. Assists resident and family with transition and adjustment to placement in facility. Assists with determination of legal status of residents upon admission, provides information and obtains copies of advance directives, assures that a resident and family wishes regarding treatment (including Do Not Resuscitate (DNR) and withholding treatment) are implemented in accordance with Company policy. Informs the resident/family of resident's personal and property rights. Provides input on social, psychological, emotional needs of resident/family to supervisor. Ensures residents have required evaluations for visual, audio, mental health and dental needs, and follows up as appropriate. Arranges residents' appointments including transportation. Knowledge, Skills, Abilities & Qualifications Previous experience with geriatric population or in long-term care setting strongly preferred. High School diploma or equivalent required: AA or Bachelor's degree preferred. Ability to interact tactfully, effectively, and professionally with other employees, residents, family members, visitors, government agencies/personnel and the general public, as needed Must be able to relate to and work with physically and/or mentally ill, disabled, elderly, emotionally upset, and at times hostile people within facility and treat them with respect and consideration regardless of their cognitive or functional level Willingness and ability to function as an effective team member through good communication and cooperation to meet departmental goals Must be able to multi-task and handle pressure Willing to work beyond normal working hours, weekends, holidays, and/or on other shifts and in other positions, when necessary Sufficient computer skills including proficiency in Microsoft Office, Outlook, and /or Internet to perform required duties Ability to exercise discretion and independent judgment, and reach logical, practical decisions Submit your application and join our award-winning team!
